We also found that this facility excelled in our long-term care rating. We awarded them an impressive grade of A+ in this category. Nursing homes that receive this type of grade in long-term care tend to provide consistent around the clock care to ensure residents are kept in good health. In addition to providing well above average levels of nursing care and other staffing, this facility administered the pneumonia vaccine to 100 percent of its patients. Vaccines are vital to keeping patients healthy. Finally, we looked at its number of hospitalizations. We found that this nursing home had 4.2 hospitalizations per one thousand long-term resident days. While this admittedly wasn't as strong as some of its other scores in this area, this statistic may be skewed for some facilities due to some of the medical complexity of patients.

This nursing home also received an A+ for its short-term care score. In our short-term care rating, we attempt to forge a valuable gauge for rehabilitation services. In this process, we look at a nursing home's scope of highly-skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. This nursing home provides more services with physical therapists and registered nurses than most nursing homes we looked at. This is usually a favorable sign. Lastly, we looked at the percentage of patients who ultimately were able to return home from this nursing home. On top of excelling in the area of physical therapy hours, we found that it fared as well as just about any nursing home in the nation with 61.6 percent of its patients returning home. Most facilities are below 50 percent in this statistic.

This facility also excelled in the area of nursing, where it received a grade of A. Few nursing homes performed better in this area. Our nursing grade is based on many subcategories, however, the primary consideration is the quantity of nurse hours spent with patients. This facility provided 6.1 hours of nursing care per patient each day, which is among the better totals in the country. A significant portion of those hours were provided by registered nurses, which are one of the more skilled levels of nurses. We were very impressed by both of these statistics. On top of providing high levels of nursing care, this place also performed well in several of the quality-based metrics we looked at. By way of illustration, in terms of the percentage of its patients suffering falls resulting in major injury, this facility performed as well as any facility the state. Preventing serious falls is typically a good indicator that a facility has reliable quality controls in place. Major falls can frequently be prevented if more nurses and better safety protocols are in place.

Another impressive feature for this nursing home is its inspection grade. In that area, we gave this nursing home an elite grade of A, wrapping up its straight A performance. Inspection ratings account for a host of factors included in a facility's inspection report. One of the most important criteria we consider is the number and severity of deficiencies. Facilities with higher grades in this category typically have few severe deficiencies. Fortunately, although this facility had some minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This means that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
