Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection ratings weigh several factors, including deficiencies and federal fines. You can learn more about each of these items by obtaining copies of nursing homes' inspection reports. While this facility had some minor dings on its report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G through L. This means CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A couple minor deficiencies aren't necessarily the end of the world.
Nurse Quality
This facility excelled in nursing, where it earned an D. Nursing scores are largely tied to the facility's level of nurse staffing. With just 0.5 hours of nursing care per resident daily, this nursing home's nurse staffing levels were well below the national average. Lastly, we also looked at some nursing quality measures in computing our nursing ratings. This nursing home fared well in the area of minimizing its residents' pressure ulcers and major falls. We consider these areas to be reliable indicators of the quality of nursing care.

Long-term Care Quality
Sadly, this facility received an F for its long-term care score. When facilities receive this kind of score in this category it is often a bad sign for patient care and it may mean that the nursing home is not as well-staffed. After looking at the amount of care provided by nurses aids and other staff, we then looked at the nursing home's vaccination record. This facility vaccinated 93.15476 percent of its residents for pneumonia, which happens to be somewhat below what we anticipated but still a respectable percentage. To our surprise, this nursing home was actually able to limit hospitalizations. With just 0.02 hospitalizations per 1,000 long-term resident days, this nursing home has fewer hospitalizations than the majority of nursing homes. This is its best score in this category.
Short-term Care Quality
The next area we graded is short-term care. This nursing home was given a very poor grade of F in this category. In forming these short-term care scores, we look at the nursing home's skilled nursing services, including those performed by registered nurses, respiratory therapists, physical therapists and other types of therapists. Our objective is to devise a measure for sizing up the rehabilitation services of various facilities. As you might expect, we discovered that this place offers far less registered nurse and physical therapist hours per resident than the typical nursing home. Finally, we looked at the percentage of patients who eventually returned home from this facility. This facility didn't fare well here either. In fact, we found that just 3 percent of this facility's patients were able to return home.
