Oxley Park Health and Rehabilitation is the lone option located in Lyons, Georgia. This nursing home is an A rated facility, which is one of the most impressive grades we offer. We were so impressed with this facility that we ranked it in the top fifth of all facilities in the nation. One of the best aspects of this facility's stellar report card is its inspection score. Facility Inspections

Grade: A-plus

One of the many reasons this turned out to be a quality nursing home is that it received an A+ inspection grade. This is simply as good as it gets in this critical category. Inspection ratings are based on several datapoints located in the a nursing home's recent inspection reports. Facilities that excel in this category typically have few deficiencies on those reports. Most importantly, these places generally do not have any severe deficiencies which are associated with risks to patient safety. This facility was assessed 4 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be severe. This indicates that CMS didn't deem any of the deficiencies to create an immediate risk to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Long-term Care Quality

Grade: A-plus

This exemplary nursing home also performed well in the category of long-term care, where we awarded it a score of A+. Very few nursing homes performed better in this category. Facilities that receive this kind of grade in this category typically provide consistent 24/7 care to ensure patients are well cared for. On top of looking at the amount of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This facility administered the vaccine to 100 percent of its patients, which is significantly higher than the average nursing home. Finally, this place was able to limit hospitalizations. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is an impressively low number.

Nurse Quality

Grade: A-minus

Among its many impressive grades, this facility received an excellent nursing grade. In fact, we gave it a grade of A- in that category. Nursing ratings are tied to levels of nurse staffing. This facility provided 0.4 hours of nursing care per resident per day. Finally, we also assessed several nursing quality-based metrics and this nursing home excelled in some of these areas. With less than five percent of its residents sustaining pressure ulcers, it performed as well as any place the country in this category. This is generally a good indicator of quality nursing care. Pressure ulcers can often be avoided by providing better nursing care, such as by employing a policy of moving patients more often.

Short-term Care Quality

Grade: B

The final category we analyzed is short-term care. We gave this facility one of our more favorable scores in this category, with a grade of B. While this was not as favorable as some of this facility's other grades, it is still an excellent grade. In the category of short-term care, we endeavor to assess indicators of a nursing home's rehabilitation services. We look at the nursing home's skilled nursing services, including those performed by registered nurses and physical, occupational and other therapists. Fortunately, it appear that this nursing home has registered nurses on staff. Not all facilities employs these skilled professionals. On the other hand, according to the information this nursing home provided, they do not appear to employ physical therapists. Finally, we assessed the number of patients that ultimately were able to return home from this nursing home. We found that 0 percent of this facility's patients returned home.
