While this nursing home's overall grade was as bad as it gets, it actually excelled in the area of inspections. In fact, we gave it a grade of A for that category, which is one of our highest scores. We weigh several aspects of a nursing home's inspection report in computing our inspection grades. One of those factors is health deficiencies. It should be noted that the severity of these deficiencies is usually more meaningful than the number of deficiencies, as some of these can be relatively insignificant. Fortunately, although this nursing home had some minor dings on its report, it had zero severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This means that the government inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. We gave this nursing home just a D for our long-term care grade and that actually ended up being its second highest category. In a long-term care setting, the primary goal is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. Once we assessed the volume of nursing care, we turned to the nursing home's vaccination record. We were pleased to find that this nursing home vaccinates 99.62963 percent of its patients against pneumonia. Surprisingly, this nursing home also fared well at keeping its residents out of the hospital. Indeed, it had just 1.24 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in this category weren't as favorable as these.

Our third area we looked at is short-term care, where this facility received an F. In calculating our short-term care grades, we quantify the nursing home's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our goal is to formulate a barometer for sizing up the rehabilitation services of different facilities. Given its grade in this area, we weren't stunned to learn that this facility is well below average in terms of the number of registered nurse and physical therapy hours offered to its patients based on the measures we assessed. The last measure we considered in this area is the number of patients who were able to leave the facility and return to the community. We discovered that just 37.5 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics doomed this facility's short-term care grade.

The final category we assessed was nursing, in which this facility was given a rock bottom grade here as well. Sadly, it received an F for this area, which is definitely very concerning. Our nursing rating is based on many subcategories. The most heavily weighted variable is the amount of time nurses spend with patients. This nursing home provided just 2.8 hours of nursing care per patient on a daily basis. This is a very low total compared to most nursing homes. To go along with its low totals in the area of nursing hours per resident, this nursing home also did not fare as well in the quality-based measures we looked at in calculating our nursing ratings. We looked at the percent of residents sustaining major falls and pressure ulcers. This nursing home had more falls and pressure ulcers than the average nursing home.
