On top of receiving a strong overall score, this nursing home also received first-rate government inspections in recent years. We awarded it one of our better grades in this category, with a grade of A+. Inspection grades are tied to many items located in the a nursing home's recent inspection reports. Nursing homes that excel in this area tend to have very few deficiencies on their reports. Most importantly, these facilities generally do not have any severe deficiencies which are associated with endangerment of patients. While this nursing home had some deficiencies on its inspection report, none were severe based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

This facility was awarded a strong long-term care score as well. We gave it one of our higher grades in that area with a grade of B+. Facilities that receive this kind of score in long-term care tend to provide consistent 24/7 care to ensure patients are kept in good health. On top of assessing the amount of care provided by aids and other staff, we looked at the number of residents vaccinated for pneumonia. This facility provided the vaccine to 77.118645 percent of its patients. This is slightly less than we anticipated. Clearly, this facility is doing something right in this area as it also excelled at keeping its residents out of the hospital. It had less than one hospitalization per 1,000 long-term resident days, which is an impressively low number.

The next highest grade we gave this nursing home came in the category of nursing, where we gave it a grade of B-. This wasn't quite as good as some of its other scores, but a grade in this range is not the end of the world. In determining a nursing home's nursing score, we assess the quantity of hours nurses are with patients and the levels of training of the nurses. This place provided just 0.3 hours of nursing care per resident per day. This is a relatively low total compared with most nursing homes. Finally, nursing ratings also factor in quality-based measures, such as preventing major falls. Despite not having the most impressive total nursing hours per resident, this place actually performed admirably in the area of preventing major falls. This is generally a good indicator that a nursing home has sufficient quality controls in place. Many falls can be avoided if a nursing home supplies enough nurses aids to assist its residents.

This nursing home's worst category was short-term care, which is the final area we scored. In this area, we gave this nursing home a grade of just D. In the category of short-term care, we attempt to evaluate indicators of a facility's rehabilitation. We assess the nursing home's skilled nursing services, such as those performed by registered nurses and various therapists. In this nursing home's case, we found that it provides less physical therapist and registered nurse hours per resident than the average facility. The final item we considered in this area is the percentage of residents who were able to leave the facility and return to the community. We discovered that just 4.3 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar metrics doomed this nursing home's short-term care score.
