Facility Inspections
Although we graded this nursing home incredibly poorly overall, it actually received a very impressive inspection report this year. As a result, it earned one of our better grades in that area with a B. Perhaps the most critical factor we look at in calculating our inspection ratings is deficiencies. Deficiencies are found on a facility's inspection reports. It is typically in your best interest to avoid facilities that have too many severe deficiencies. This nursing home received 6 deficiencies on its inspection report, but none were considered severe. This tells you that CMS did not deem any of these deficiencies to cause an imminent risk to patient health or safety. A few minor deficiencies aren't necessarily the end of the world.
Short-term Care Quality
Turning to the area of short-term care, this facility was given just a D in that area. Unfortunately, this ended up being its second best grade. In computing our short-term care grades, we scrutinize a facility's levels of skilled nursing, which includes registered nurses, physical therapists occupational therapists and other highly trained professionals. This score is more often than not a fair measure of the facility's rehabilitation. Based on its abysmal score in this category, we weren't shocked to find this nursing home offered far fewer physical therapist and registered nurse hours per patient than most facilities. Finally, we considered the number of patients who returned home from this facility. This facility didn't fare well here either. In fact, we found that just 0 percent of this facility's patients were able to return home. This figure was well off the national average.

Nurse Quality
We also wanted to point out that this nursing home received an F for its nursing grade. Our nursing rating looks at the nursing home's nurse staffing levels. We factor in both the levels of licensure of those nurses and the quantity of hours spent with patients. With only 0.3 hours of nursing care per resident each day, this nursing home's nurse staffing levels were well below average. Finally, we also assessed some nursing quality-based metrics in computing our nursing scores. This nursing home performed well in the area of minimizing its residents' pressure ulcers and major falls.
Long-term Care Quality
The final area we rated is long-term care. This nursing home was given a rock bottom grade of F in that area. For patients looking for a permanent place to live as opposed to rehabilitation, long-term care grades are very important. After assessing the amount of care provided by aids and other staff, we next considered the nursing home's vaccination records. This nursing home vaccinated 100 percent of its residents against pneumonia, which is higher than most nursing homes. To our surprise, this place also fared well at keeping its residents out of the hospital. Indeed, it had just 0 hospitalizations per one thousand long-term resident days. Sadly, a few of its other scores in the category were not as favorable as these.
