We want to draw your attention to the fact that this facility really outperformed its overall grade in the category of inspections. In fact, it received an excellent inspection report this year. As a result, it received one of our best grades in that category with a grade of A. Our inspection scores are tied to many datapoints located in the a nursing home's recent inspection reports. Facilities that score well in this category typically have very few deficiencies on their reports. Most importantly, these places should not have any severe deficiencies which are associated with endangerment of patients. This nursing home was hit with 2 deficiencies on its inspection report, but we were relieved to see that none were considered to be severe deficiencies. This means that the inspectors didn't consider any of these deficiencies to create an imminent risk to resident health or safety. A few minor deficiencies should not lead you to cross a nursing home off your list.

This nursing home also excelled in the area of nursing, where it received an B. There are a number of datapoints within this area. Most of these datapoints reflect staffing levels. This nursing home provides 3.8 hours of nursing care per resident per day. Finally, we also looked at several nursing quality-based metrics and this nursing home excelled in some of these areas. With less than five percent of its residents sustaining pressure ulcers, it performed as well as any place the state in this category. This is generally a good indicator of quality nursing care. Pressure ulcers can many times be avoided by providing better nursing care, such as by employing a system of turning residents even once per day.

We gave this nursing home a grade of just D for our long-term care rating. This is not one of our better scores. In a long-term care environment, the nursing home's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. After we finished looking at the quantity of care provided by nurses, we then looked at the nursing home's vaccination data. We were optimistic to find that this nursing home vaccinates 100 percent of its patients for pneumonia. Surprisingly, this facility also fared well at keeping its patients out of the hospital. It had only 1.2 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in this category were not as favorable as these.

The next area we assessed was short-term care, proved to be this nursing home's weakest category. We awarded this nursing home a grade of D in that category. In computing our short-term care ratings, we quantify the facility's skilled nursing services, such as those performed by registered nurses, speech therapists, physical therapists and other types of therapists. The objective is to formulate a tool for comparing the rehabilitation services of different facilities. Considering its score in this area, we weren't surprised to find that this place is well below average in terms of the quantity of physical therapy and registered nurse hours offered to its residents based on the data we assessed. Frankly, this was about what we expected to find. The final statistic we assessed in this category is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 32.5 percent of this facility's residents were able to return home, which is well below average. The combination of these poor statistics doomed this nursing home's short-term care score.
