In addition to receiving a strong overall grade, this facility performed well in our long-term care category. In fact, we gave it an impressive grade of A for that category, which is one of our highest scores. Facilities that excel in this category tend to provide residents with more supervision and stay on top of routine medical care. In addition to looking at the volume of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This nursing home provided the vaccine to 100 percent of its patients, which is an impressive figure. Pneumonia is too frequently a deadly illness for nursing home patients so we like it when a facility does not take any chances. Clearly, this facility is doing something right in this area as it also excelled at keeping its residents out of the hospital. Indeed, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low figure.

We also wanted to emphasize this nursing home's nearly flawless health inspections recently. We gave them an impressive grade of A in this area. Perhaps the most important factor we consider in calculating our inspection ratings is deficiencies. Deficiencies can be found on a nursing home's recent inspection reports. Places with better grades in this category typically avoided the more severe deficiencies involving things like patient abuse. Fortunately, although this nursing home had a few minor dings on its report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G through L. This means that CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

This facility's next best category was its nursing grade, where it received a grade of B-. While this didn't quite match its scores in some of the categories discussed above, this is still a decent score. In computing our nursing ratings, we look at both staffing levels and the levels of licensure of those nurses. This facility averages just 0.4 hours of nursing care per patient per day, which is a very low figure. Lastly, we also looked at some nursing quality measures. While this place got dinged a bit for its nursing hours, it actually excelled in a few of these quality metrics. With less than five percent of its residents suffering from pressure ulcers, it performed remarkably well in this subcategory. This is typically an indicator of quality nursing care. Pressure ulcers can many times be prevented by offering better nursing care, such as employing a policy of turning patients even once per day.

Switching gears to the next area, we awarded this facility a grade of D for short-term care. This is the nursing home's weakest category score. Naturally, this grade pulled down the nursing home's overall grade a bit. In the category of short-term care, we try to evaluate indicators of a facility's rehabilitation. We look at the facility's skilled nursing services, including those performed by registered nurses and physical, occupational and other therapists. In this nursing home's case, we found that it provides less physical therapist and registered nurse hours per resident than the average facility. The final measure we assessed in this category is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 4.3 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics was damning for this facility's short-term care score.
