Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. These inspection scores take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can find more information about each of these items by reviewing copies of nursing homes' inspection reports. This particular nursing home received 6 deficiencies on its inspection report. Overall, this is still a solid inspection report, but we'd always recommend looking into any deficiencies you find troubling.
Long-term Care Quality
Moving on to the area of long-term care, this facility was awarded a grade of just D in this category. Unfortunately, this wound up being one of its best areas. For prospective patients seeking a permanent place to live as opposed to skilled nursing, long-term care grades are very important. Once we looked at the volume of care provided by nurses and other staff, we then looked at the nursing home's vaccination data. This facility vaccinated 99.03226 percent of its residents against pneumonia, which is higher than the majority of nursing homes. Surprisingly, this place was able to limit hospitalizations. With only 0.01 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than the majority of nursing homes. Sadly, a few of its other scores in this category weren't as favorable as these.

Nurse Quality
Turning to the area of nursing care, this facility received a grade of just D in this category. When calculating a nursing home's nursing rating, we consider the quantity of hours nurses are with residents as well as the level of licensure of the nurses. This nursing home offered just 0.4 hours of nursing care per resident on a daily basis. This is not a very impressive figure. Finally, we also assessed some nursing quality-based metrics in calculating our nursing scores. This nursing home performed well in the area of minimizing its residents' pressure ulcers and major falls.
Short-term Care Quality
The next area we scored was short-term care, where this nursing home received an abysmal grade here also. Sadly, it received an abysmal F for this category, which is obviously very concerning. In calculating our short-term care scores, we scrutinize a nursing home's levels of highly skilled nursing services, which includes registered nurses, physical therapists speech therapists and other highly trained professionals. This category is considered to be a reliable assessment of the nursing home's rehabilitation services. Considering its grade in this category, we weren't surprised to discover that this place is well below average in terms of the quantity of physical therapy and registered nurse hours provided to its residents based on the metrics we assessed. Honestly, this was what we expected to find. The final measure we assessed in this category is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 4.6 percent of this facility's residents were able to return home, which is well below average. The combination of these poor metrics doomed this nursing home's short-term care grade.
