Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. We believe that one of the most important factors in assessing an inspection report is deficiencies. In this regard, we especially focus on the severity of those deficiencies. This nursing home was assessed 4 deficiencies on its inspection report, but we were relieved to find that none were category G through L deficiencies. This means CMS did not consider any of the deficiencies to create an imminent danger to resident safety or health. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.
Nurse Quality
We were also surprised to find that this facility received a decent nursing grade this year. We gave it a grade of C in that category. Our nursing rating is primarily associated with a facility's level of nurse staffing. This facility provides 0.3 hours of nursing care per resident each day. This is a much lower figure than we are used to seeing. Finally, despite not having the highest total nursing hours per resident, this facility actually performed admirably in the area of preventing major falls. This is usually an indicator that a nursing home has quality controls in place.

Short-term Care Quality
Unfortunately, this facility only received a grade of D for its short-term care grade, which is not a score to write home about. Short-term care grades are vital for prospective residents requiring rehabilitation. Rehabilitation generally mandates higher levels of highly-skilled nursing services. With a score of this caliber for this category, we weren't stunned to learn this place was below average in multiple staffing metrics we assess. The facility supplied substantially fewer physical therapist and registered nurse hours per patient than most other nursing homes. Finally, we looked at the number of patients who were able to return to the community from this facility. This was not a strength for this facility. We found that just 4.6 percent of this facility's residents returned home. At most facilities, around half of their residents are able to return home so this is well below average.
Long-term Care Quality
The final category we assessed is long-term care. This facility was given a bottom of the barrel grade of F in that category. In a long-term care setting, the nursing home's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. One of the factors we considered in addition to nurse's aid hours was vaccinations.
