Facility Inspections
The primary reason this nursing home received a decent overall grade is its excellent inspections in recent years. We awarded it one of our higher scores in that category, with an A. This is a notably better grade than the facility's overall grade, which was decent but certainly not elite. We take several aspects of a facility's inspection report into consideration in calculating these inspection ratings. One critical factor is health deficiencies. It should be noted that the severity of deficiencies is usually more meaningful than the quantity of deficiencies, as some deficiencies end up being relatively minor. While this facility had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This tells you that the government inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is always a good sign.
Long-term Care Quality
In addition, we awarded this facility a grade of B+ in our long-term care category. This is one of our more impressive grades. For long-term care residents, the primary goal is to maintain patients' quality of life and keep them safe. After considering the volume of care provided by nurses and other staff, we then looked at the nursing home's vaccination record. This facility vaccinated 84.12017 percent of its residents for pneumonia, which is somewhat below what we expected but still a decent figure. While its vaccination rate was a bit lower than we had hoped, this place was able to limit hospitalizations. It had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low figure.

Nurse Quality
The next area we looked at was nursing, where we gave this facility an F. This was definitely one of this facility's major weaknesses. The nursing grade is based on many subcategories. The most heavily weighted one is the quantity of hours nurses spend with patients. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also assessed some nursing quality measures in calculating our nursing grades. This facility fared well in terms of avoiding pressure ulcers and major falls.
Short-term Care Quality
The last area we scored is short-term care. Sadly, it received an F for this category, which is a bottom of the barrel score. This is clearly a major disappointment. Short-term care grades are commonly used to gauge a facility's rehabilitation services. To offer highly graded rehabilitation services, facilities generally need to provide better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other types of therapists. As you might expect, we found that this facility offers far less physical therapist and registered nurse hours per patient than the average nursing home. Finally, we assessed the percentage of residents who were able to return to the community from this facility. We found that just 4.1 percent of this nursing home's patients returned home.
