While we gave this facility a terrible overall score, we gave it a B+ for our inspections category. This was due to the nursing home receiving a quality inspection report this year. Inspection scores account for several factors found on a nursing home's inspection reports. One of the most important criteria we consider is the quantity and severity of deficiencies. You should avoid facilities with a list of deficiencies flagged. While this place had some deficiencies on its inspection report, none were major deficiencies based on CMS' deficiency scale. A few minor deficiencies aren't the end of the world.
Short-term Care Quality
We awarded this facility just a D for our short-term care score and that actually ended up being its next most favorable category. In the category of short-term care, we attempt to assess measures of a facility's rehabilitation. We assess the nursing home's skilled nursing services, such as the ones performed by registered nurses and various therapists. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The final item we considered in this area is the percentage of residents that eventually returned home from the facility. This nursing home struggled quite a bit in this area as well, with just 4.6 percent of its residents returning home. Unfortunately, this was well below the national average.

Another unfortunate development was the F this nursing home was given in the area of long-term care. For prospective residents in need of a permanent residence as opposed to rehabilitation, long-term care grades are very important. Once we assessed the quantity of care provided by nurses, we next considered the nursing home's vaccination record. We were optimistic to discover that this nursing home vaccinates 99.25373 percent of its residents against pneumonia. To our surprise, this place also keeps its residents out of the hospital. Indeed, it had only 0 hospitalizations per one thousand long-term resident days. Sadly, some of its other scores in the category were not as favorable as these.
Nurse Quality
The next area we rated was nursing, where this facility was given a very poor grade in this area also. It received a lowly F in this area, which is definitely somewhat alarming. When computing our nursing scores, we weigh both the number of hours nurses spend with residents and the levels of licensure of the nurses. This place provided just 0.3 hours of nursing care per patient each day. Unfortunately, this facility also had abysmal marks in some of the quality-based measures to go along with its weak nursing hour totals. We looked at the percentage of residents sustaining pressure ulcers. This facility was at more than double the national average in this datapoint. This is quite alarming when you consider that many bed sores are believed to be preventable with better nursing care. This ended up sealing the nursing home's fate of earning a weak nursing rating.
