Neodesha Care & Rehabilitation Center is the only facility we found in Neodesha, Kansas. With an overall rating of F, we consider this to be a bottom of the barrel nursing home. This facility doesn't seem to have much going for it. Finally, there was an ownership change at this facility in the last year. Ideally, their performance will improve under new ownership.

Facility Inspections
While we graded this nursing home incredibly poorly overall, it received a very impressive government inspection report this year. As a result, it received one of our best scores in that area with a grade of A-. These inspection scores weigh several factors, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by obtaining copies of nursing homes' inspection reports. Fortunately, although this place had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This means that the inspectors did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A couple minor deficiencies aren't the end of the world.
Short-term Care Quality
Unfortunately, this facility received an F for its short-term care score. Our short-term care grades are presumably more critical for residents needing rehabilitation services. Rehabilitation generally utilizes higher levels of highly-skilled nursing. Skilled nursing includes not only nursing services, but also physical and occupational therapy, as well as other variations of therapy. Given its abysmal grade in this category, we weren't surprised to find this nursing home provided substantially fewer physical therapist and registered nurse hours per resident than most other nursing homes. The final statistic we looked at in this area is the number of residents who were able to leave the facility and return to the community. We discovered that just 26.9 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics doomed this nursing home's short-term care grade.

In addition, we gave this facility an F in the area of nursing care. There are a host of subcategories included in this area. Many of the factors relate to nurse staffing. This nursing home offered a meager 2.8 hours of nursing care per patient per day. This number is significantly below the national average. To pair with its subpar totals in the area of nursing hours per patient, this nursing home also didn't fare as well in the quality-based measures we looked at in determining our nursing grades. We looked at the percent of residents sustaining pressure ulcers and major falls. This facility had more pressure ulcers and falls than the average facility. This could be a bad sign when you consider that many falls and bed sores are believed to be preventable with better nursing care. These scores hurt this nursing home's nursing grade quite a bit.
Long-term Care Quality
The final category we rated is long-term care. This facility was given an abysmal grade of F in this category. In a long-term care environment, the nursing home's primary objective is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. After assessing the volume of care provided by aids and other staff, we then analyzed the nursing home's vaccination statistics. This nursing home vaccinated 95.48872 percent of its patients for pneumonia, which is better than most nursing homes. To our surprise, this place was able to limit hospitalizations. With just 1.08 hospitalizations per 1,000 long-term resident days, this nursing home has fewer hospitalizations than the average nursing home. Unfortunately, a few of its other scores in this category weren't as favorable as these.
