Facility Inspections

Grade: A

One of the reasons this turned out to be a great nursing home is it received a very impressive grade in inspections. Inspections turned out to be its best category. In that area, we awarded this facility an A. Perhaps the most significant factor we consider in determining our inspection scores is deficiencies. Deficiencies are found on a nursing home's recent inspection reports. Places with better grades in this area typically avoided the more severe deficiencies involving health risks or even death. Fortunately, although this facility had a few minor dings on its report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This means CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Long-term Care Quality

Grade: A-minus

This nursing home also received a dominant long-term care score. In fact, this turned out to be this facility's second strongest category score. In that area, we awarded this facility a score of A-. Nursing homes that receive this kind of grade in long-term care tend to provide consistent around the clock care to make sure patients are kept in good health. One of the criteria we considered after nursing hours is vaccinations. This facility provided the pneumonia vaccine to 96.27907 percent of its patients. Vaccination is a proven method to avoid negative health outcomes for the nursing home population. Clearly, this nursing home is doing something right in this area as it was able to keep its patients out of the hospital. Finally, our nursing grades also factor in quality-based assessments, such as avoiding major falls. This nursing home performed well in this area. Avoiding major falls is typically a good indicator that a place has reliable quality controls in place. Major falls can many times be prevented if more nurses aids and better safety protocols are in place.

Short-term Care Quality

The last category we graded was short-term care, where this nursing home was awarded a grade of B. Believe it or not, this proved to be this nursing home's worst category. A facility is doing something right when it's worst grade still is superior to most facilities. Short-term care grades are commonly employed to gauge a facility's performance with rehabilitation. In order to have highly scored rehabilitation services, facilities generally must offer better levels of highly skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other highly trained professionals. It doesn't look like this facility submitted information on registered nurse or physical therapist staffing. Finally, we assessed the number of patients that who were able to eventually return home from this facility. We found that 5 percent of this facility's residents returned home.
