Moravian Hall Square Health and Wellness Center is a small non-profit nursing home located in Nazareth, Pennsylvania. In addition to receiving an elite overall rating, this facility earned an A+ inspection rating. This is based on the facility's recent inspection reports. An A+ in this area is one of the highest complements we can offer a nursing home. We weigh several aspects of a nursing home's inspection report in calculating our inspection ratings. One critical factor is health deficiencies. It should be noted that the severity of deficiencies is usually more important than the number of deficiencies, as some deficiencies can be relatively insignificant. While this facility had a few minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This tells you the inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
In addition, this nursing home received elite marks in the area of long-term care. For this reason, it was given one of our best scores in that area with a grade of A+. Nursing homes that do well in this category tend to be well-staffed and offer extensive hands on care to patients. On top of assessing the well above average volume of care provided by nurses and other staff at this nursing home, we also were happy with the facility's record for vaccinations. By way of example, this nursing home provided the pneumonia vaccine to 100 percent of its patients, which is what we like to see. This combination proved to be effective as this nursing home keeps its patients out of the hospital. Indeed, it had just 1.38 hospitalizations per 1,000 long-term resident days.

Another reason we rated this nursing home so highly is that it received an A+ nursing grade. This is simply as good as it gets in this category. Our nursing grade is based on the facility's level of nurse staffing. We factor in both the levels of skill of the nurses and the number of hours spent with residents. This facility offers extremely high levels of nursing care, averaging 4.6 hours per resident on a daily basis. This is more nursing care than nearly any other nursing home provides. Lastly, this nursing home also excelled in several of the quality-based metrics we looked at. With fewer than five percent of its residents suffering from pressure ulcers, it performed as well as any nursing home the country in this category.
Short-term Care Quality
Moving on to our next category, this facility also performed well in our short-term care category. We gave it a grade of A for this area. This wrapped up truly elite report card. In calculating our short-term care grades, we look at the nursing home's skilled nursing services, including those performed by registered nurses, respiratory therapists, physical therapists and other types of skilled professionals. Our purpose is to devise a barometer for comparing the rehabilitation services of various facilities. This nursing home provides more services with physical therapists and registered nurses than most facilities we looked at. This is generally a very favorable sign. The final datapoint we assessed in this category is the percentage of patients who were able to return home from the facility.
