While this facility's overall grade was as bad as it gets, it actually excelled in the category of inspections. In fact, we gave it an A- for that category, which is one of our highest scores. Inspection grades weigh several factors, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these factors by obtaining copies of nursing homes' inspection reports. While this facility had a few deficiencies on its government inspection report, none were severe based on CMS' deficiency scale. A couple minor deficiencies are not the end of the world.

We were also stunned to learn that this facility received a somewhat respectable long-term care rating in our assessment. We awarded it a C in this area. In calculating these long-term care scores, we analyze the nursing home's personal care services. On top of looking at the quantity of care provided by aids and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This facility gave the vaccine to 98.82353 percent of its patients. The last statistic we assessed is the nursing home's hospitalization rate. Although it had 2.03 hospitalizations per 1,000 long-term resident days, its risk adjusted score was not too bad.

The third area we analyzed is short-term care. This facility's received a low-end short-term care score where it was given a lowly F. Short-term care grades are meaningful for prospective residents seeking rehabilitation. Rehabilitation usually requires more highly-skilled nursing. Skilled nursing includes a broad spectrum of services, spanning from registered nurses to physical and respiratory therapists, as well as other forms of therapy. With a grade this poor in this area, we were not surprised to find that this place was below average in the two key staffing areas we focus on. It supplied significantly fewer registered nurse and physical therapist hours per resident than most other nursing homes. The final metric we looked at in this area is the number of residents who ultimately returned home from the facility. This nursing home struggled quite a bit in this metric as well, with just 39.2 percent of its patients returning home.

The last category we looked at was nursing. This nursing home was given a rock bottom grade of F in that area. The nursing grade assesses many components, but the main consideration is the quantity of nurse hours spent with patients. This nursing home averaged a meager 3.1 hours of nursing care per patient per day. To pair with its low totals in the area of nursing hours per resident, this place also didn't fare as well in the quality-based measures we looked at in determining our nursing ratings. We looked at the percentage of residents experiencing pressure ulcers and major falls. This nursing home had more pressure ulcers and falls than the average nursing home.
