Facility Inspections

Grade: B-plus

Although this nursing home's overall grade was not a highlight, it actually received a very good grade in the category of inspections. We gave it a grade of B+ for that area, which is one of our better scores. These inspection grades take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these factors by reviewing copies of nursing homes' inspection reports. This facility was hit with 5 deficiencies on its inspection report, but fortunately none were considered severe. This tells you that the inspectors did not deem any of these deficiencies to be an immediate risk to patient health or safety. A few minor deficiencies are not necessarily the end of the world.

Short-term Care Quality

Grade: C

This facility's short-term care score turns out to be one of its higher scores. The facility was given a C in this category. Our short-term care grades are critical for residents needing rehabilitation. Rehabilitation usually mandates more skilled nursing services. Skilled nursing includes a broad range of services, spanning from registered nurses to physical and occupational therapists, as well as other types of therapy. Unfortunately, this facility was below average in terms of its number of physical therapy and registered nurse hours provided to its patients based on the metrics we assessed. The final measure we looked at in this area is the percentage of residents that who were able to eventually return home from the nursing home. We found that just 4.8 percent of this facility's patients returned home.

Long-term Care Quality

Grade: F

This facility wound up receiving a bottom of the barrel grade in our long-term care category. It received a lowly F in this category. If you are looking for services other than short-term rehabilitation, you should take a close look at long-term care grades. In addition to assessing the quantity of care provided by nurses and other staff, we looked at the number of residents vaccinated for pneumonia. This nursing home administered the vaccine to 100 percent of its residents, which is better than the majority of nursing homes. Surprisingly, this place also keeps its residents out of the hospital. In fact, it had just 0.04 hospitalizations per one thousand long-term resident days. Unfortunately, some of its other scores in this category weren't as favorable as these.

Nurse Quality

Grade: F

Turning to our final category, this nursing home didn't perform well in this area either. With a bottom of the barrel grade of F in nursing, this is just about as as uninspiring as it gets. Our nursing grade focuses on the facility's level of nurse staffing. We weigh both the levels of training of those nurses and the number of hours spent with patients. With only 0.3 hours of nursing care per resident per day, this facility's nurse staffing levels were well below average. Unfortunately, this place also had terrible scores in several of the quality-based metrics to pair with its weak nursing hours. For starters, we looked at the percentage of residents experiencing pressure ulcers. This facility was at more than twice the national average in this datapoint. This is quite alarming when you consider that so many bed sores are believed to be preventable with better nursing care. This ended up sealing the facility's fate of receiving a poor nursing grade.
