Long-term Care Quality
This facility really performed well in our long-term care category. We awarded it an A- for that category. For long-term care residents, the nursing home's primary goal is to maintain patients' quality of life and keep them safe. After considering the volume of care provided by aids and other staff, we turned to the facility's vaccination record. This nursing home vaccinated 55.339806 percent of its patients against pneumonia, which happens to be a bit below what we anticipated but still a respectable figure. Although its vaccination rate was a bit lower than we had hoped, this facility was able to limit hospitalizations. It had less than one hospitalization per one thousand long-term resident days, which is a remarkably low number.
Facility Inspections
This facility also performed well in the area of inspections. As a result, it received one of our best scores in that category with a score of B+. Our inspection grades are based on datapoints located in the a nursing home's inspection reports. Places that excel in this category tend to have very few deficiencies on their reports. Most importantly, these nursing homes should not have any severe deficiencies which are associated with risks to patient safety. Fortunately, although this place had some minor dings on its report, it had no severe deficiencies. Severe deficiencies are found in categories G through L. This means the inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Nurse Quality
The next highest grade we gave this nursing home in any area came in the category of nursing, where we gave it a grade of C. Our nursing category weighs many factors, most of which are associated with nursing hours. This nursing home averages just 0.4 hours of nursing care per resident per day. This is a very low figure. Despite having low quantities of nursing care, this nursing home fared better in some of the quality-based metrics we looked at. In fact, it performed well when it comes to avoiding major falls and pressure ulcers.
Short-term Care Quality
The last category we looked at was short-term care. We gave this facility a D for the area of short-term care, rendering this its least impressive category grade. Our short-term care grade is typically used to gauge a facility's rehabilitation services. To have quality rehabilitation services, facilities generally need to feature better levels of skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other licensed professionals. In this facility's case, we found that it provided less registered nurse and physical therapist hours per resident than a typical facility. The last metric we looked at in this category is the number of residents who were able to leave the nursing home and return to the community. We discovered that just 5.2 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar statistics sunk this nursing home's short-term care grade.
