Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. We would argue that one of the most critical factors in evaluating an inspection report is deficiencies. We especially look at the severity of the deficiencies. To our surprise, this nursing home had a category J through K deficiency, which are some of the worst deficiencies. This often means that the government inspectors uncovered a deficiency which places patient health or safety in imminent danger. In addition to having severe deficiencies, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Long-term Care Quality
Incredibly, we awarded this facility a C for our long-term care grade, which really isn't a bad grade. In a long-term care setting, the nursing home's primary goal is to maintain patients' quality of life and keep them safe. After considering the volume of care provided by aids and other staff, we then analyzed the facility's vaccination records. We were concerned by the fact that this nursing home vaccinated just 44.67213 percent of its patients for pneumonia. Nevertheless, this facility excelled at keeping its residents out of the hospital. It had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low number.

Nurse Quality
We gave this facility a grade of just D for our nursing rating. Our nursing grade consists of quite a few factors, however, the main consideration is the quantity of nurse hours per patient per week. With just 0.3 hours of nursing care per resident daily, this facility's nurse staffing levels were significantly below the national average. Lastly, we also assessed some nursing quality-based metrics in determining our nursing grades. This place performed well when it comes to minimizing its residents' major falls and pressure ulcers.
Short-term Care Quality
This facility's least impressive area was short-term care, which is the final area we scored. In this category, we gave this nursing home a grade of D. Our short-term care ratings are based in part on a facility's quantity of highly-skilled nursing services. This includes a broad spectrum of services, ranging from registered nurses to physical and occupational therapists, as well as other types of therapy. With its score in this area, we weren't surprised to discover that this nursing home is well below average in terms of its quantity of registered nurse and physical therapy hours provided to its patients based on the metrics we looked at. Finally, we considered the percentage of patients that ultimately returned home from this facility. This nursing home didn't fare well here either. In fact, we found that just 4.4 percent of this facility's residents were able to return home. This figure was well off the national average.
