Facility Inspections
While we did not have many favorable things to say about this place's overall grade, it earned a somewhat respectable government inspection report this year. As a result, it received a decent grade in that category with a B-. We believe that one of the most important factors in evaluating an inspection report is deficiencies. In this regard, we especially focus on the severity of the deficiencies. Although this nursing home had some deficiencies on its inspection report, none were severe based on CMS' deficiency scale. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Nurse Quality
We also want to point out that this facility received a decent grade in our nursing category. We awarded it a grade of B- in the area. Nursing ratings are primarily based on the facility's nurse staffing. This nursing home provides only 0 hours of nursing care per patient daily. This is a somewhat alarming figure as it is well below average. Finally, we also baked a few quality-based metrics into our nursing ratings. This nursing home was relatively weak in two of the major areas we look at, with below average scores for minimizing its residents' major falls and pressure ulcers. These areas are generally reliable indicators of the quality of nursing care offered.

Short-term Care Quality
The third area we graded was short-term care, where this nursing home received a B- in this category. With our short-term care score, we try to craft a meaningful barometer for rehabilitation services. In this process, we assess the nursing home's level of skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. Unfortunately, we found that this nursing home provided fewer registered nurse and physical therapist hours per resident than most other facilities. The more important thing is determining what effect this had on the quality of care. Finally, we assessed the number of patients who were able to return to the community from this nursing home. This was a much stronger area for this facility. We found that it outperformed most facilities in this metric with 58.2 percent of its patients returning home. This really bolstered its score in this area.
Long-term Care Quality
The final category we scored was long-term care, in which this nursing home was given a very poor score here as well. It received an abysmal F for this category, which is definitely somewhat alarming. Facilities that receive this type of score in long-term care likely don't provide the kind of consistent around the clock care that some other nursing homes provide. In addition to considering the amount of care provided by nurses aids and other staff, we looked at the percentage of residents vaccinated for pneumonia. As it turns out, this facility administered the vaccine to just 82.87037 percent of its residents, which is somewhat concerning. Unfortunately, this place's hospitalization rate was also disappointing. We found that this facility had 2.52 hospitalizations per 1,000 long-term resident days, which is an alarming rate.
