This facilities grades were terrible across the board. In particular, we wanted to draw your attention to its poor nursing grade where it received an F. Our nursing category is based on many components, many of which are tied to levels of nurse staffing. This facility provided just 2.7 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. In addition to its subpar nursing hours, this place also received suboptimal marks in several of the quality-based measures we looked at in computing our nursing scores. We found that a higher percentage of this facility's residents experienced falls which led to major injury.

In this process, we analyze the nursing home's offerings of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Given its grade in this area, we weren't stunned to learn that this place was well below average in terms of its number of registered nurse and physical therapy hours provided to its patients based on the metrics we assessed. Honestly, this was what we expected here. The final item we looked at in this area is the percentage of patients that returned home from the facility. This facility struggled quite a bit in this area as well, with just 41.1 percent of its residents returning home. This was well below the national average.

The final area we rated was long-term care, where this nursing home was given a rock bottom grade here as well. We gave it a lowly F for this category, which is obviously a major concern. For patients looking for a permanent place to live as opposed to rehabilitation, long-term care grades are a key measure. On top of considering the quantity of care provided by aids and other staff, we analyzed the number of residents vaccinated against pneumonia. This facility administered the vaccine to 100 percent of its residents. Surprisingly, this nursing home was able to limit hospitalizations. With just 0.92 hospitalizations per 1,000 long-term resident days, this nursing home had less hospitalizations than many nursing homes. Unfortunately, some of its other scores in this category were not as favorable as these.
