Facility Inspections
While this facility's overall grade was as bad as it gets, it actually excelled in the category of inspections. In fact, we awarded it an A- for that category, which is one of our highest scores. Our inspection scores are based on several datapoints located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these inspections. You should especially avoid nursing homes with severe deficiencies associated with endangerment of patients. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Despite this deficiency, this place still had a relatively favorable report.
Short-term Care Quality
This facility's second most favorable score was short-term care, but even there it earned a grade of just D in that category. This still happens to be a relatively poor grade. Our short-term care grades are probably most meaningful for residents needing rehabilitation from their nursing home. Rehabilitation generally utilizes higher levels of skilled nursing. Skilled nursing includes not only nursing services, but also physical and respiratory therapy, as well as other variations of therapy. Based on its score in this category, we weren't surprised to learn that this place was well below average in terms of the quantity of physical therapy and registered nurse hours provided to its patients based on the metrics we looked at. Frankly, this was about what we expected to find. The last item we considered in this category is the percentage of residents who were able to leave the nursing home and return to the community. We discovered that just 0 percent of this facility's residents were able to return home, which is well below average. The combination of these poor statistics sunk this nursing home's short-term care score.

Long-term Care Quality
Additionally, we gave this facility a rock-bottom F in the area of long-term care. For residents seeking a permanent residence as opposed to skilled nursing, long-term care grades are very important. After looking at the amount of care provided by nurses aids and other staff, we next considered the facility's vaccination data. We were optimistic to learn that this facility vaccinated 99.44752 percent of its residents for pneumonia. Surprisingly, this nursing home was able to limit hospitalizations. With just 0.01 hospitalizations per 1,000 long-term resident days, this facility had fewer hospitalizations than many nursing homes. Unfortunately, some of its other scores in this category weren't as favorable as these.
Nurse Quality
The next area we graded was nursing, in which this facility was given a bottom of the barrel score in this category as well. Sadly, it received a lowly F in this category, which is obviously a major disappointment. When determining our nursing scores, we factor in both staffing levels and the levels of licensure of the nurses. This nursing home provided only 0.3 hours of nursing care per resident on a daily basis. This is a very low total compared to most nursing homes. In addition to its subpar nursing hour totals, this nursing home also received suboptimal scores in many of the quality-based metrics we considered in computing our nursing ratings. We found that more of this place's patients experienced falls leading to major injury.
