Facility Inspections
While we were not complementary of this place's overall performance, it did receive an impressive inspection report this year with few dings from the applicable government inspectors. As a result, it received one of our better grades in that area with a grade of B+. We weigh several aspects of a nursing home's inspection report in determining our inspection grades. One critical factor is health deficiencies. It should be noted that the severity of these deficiencies is arguably more important than the number of deficiencies, as some of these are relatively minor. Although this place had a few deficiencies on its report, none of them were serious based on CMS' scale. A few minor deficiencies aren't the end of the world.
Short-term Care Quality
This nursing home's next most favorable category was short-term care. We awarded it a grade of C in this category. This is basically a middle of the road grade in this category. Short-term care grades are often used to assess a facility's rehabilitation services. To offer highly scored rehabilitation services, nursing homes generally must feature better levels of highly skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other types of therapists. Unfortunately, this facility was below average in terms of its level of registered nurse and physical therapy hours provided to its residents based on the measures we assessed. The important question is whether this adversely affected quality of care. Finally, we looked at the percentage of residents who were able to return to the community from this nursing home. We found that 5 percent of this facility's residents returned home. At most facilities, closer to half of their patients are able to return home so this figure is below average.

Nurse Quality
This facility's next best category was nursing, but even there it received a grade of just D. This is still a well below average grade. Our nursing score assesses a handful of subcategories, however, the most important consideration is the quantity of nurse hours spent with patients. With just 0.3 hours of nursing care per resident daily, this facility's nurse staffing levels were significantly below average. Lastly, we also looked at some nursing quality measures in determining our nursing scores. This facility performed well when it comes to minimizing its residents' major falls and pressure ulcers.
Long-term Care Quality
This nursing home's least impressive area was long-term care, which is the final area we looked at. For this category, we gave this facility a grade of just D. If you are looking for anything other than rehabilitation, you should take a close look at each facility's long-term care ratings. On top of considering the volume of care provided by nurses and other staff, we looked at the number of residents vaccinated against pneumonia. Fortunately, this nursing home administered the vaccine to 99.646645 percent of its patients, which is a very solid total. To our surprise, this nursing home was able to keep its patients out of the hospital. Indeed, it had just 0 hospitalizations per one thousand long-term resident days. Unfortunately, some of its other scores in the category were not as favorable as these.
