Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. Our inspection scores weigh a host of factors found on a nursing home's inspection reports. One of the most important criteria we look at is the number and severity of deficiencies. This particular nursing home received 9 deficiencies on its inspection report. We are also concerned by the fact that this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.
Nurse Quality
One of the other reasons we rated this nursing home so highly is that it received a very impressive nursing score. This proved to be its second best category grade. In that area, we gave this facility a grade of C. We weighed the skill-level of nurses employed by the nursing home, as well as the number of hours those nurses spent with residents, in determining our score in this area. This particular nursing home provided just 0.4 hours of nursing care per resident on a daily basis. This figure is significantly below average. Finally, despite not having the highest total nursing hours, this facility performed admirably in the metric of preventing major falls. This is usually an indicator that a nursing home has quality controls in place.

Long-term Care Quality
This nursing home's second lowest category was long-term care, where it earned a grade of just D in that category. This is a relatively poor score. For residents seeking a permanent residence rather than rehabilitation, long-term care grades are an important measure. Once we concluded our assessment of the volume of nursing care, we then looked at the nursing home's vaccination data. We were optimistic to learn that this facility vaccinated 97.54386 percent of its patients against pneumonia. To our surprise, this facility was able to limit hospitalizations. With only 0.06 hospitalizations per one thousand long-term resident days, this nursing home had less hospitalizations than the average nursing home. Sadly, a few of its other scores in the category were not as favorable as these.
Short-term Care Quality
The next category we looked at was short-term care, was this nursing home's worst area. We awarded this nursing home a D in this category. In computing our short-term care ratings, we size up the facility's levels of highly skilled nursing services, which includes registered nurses, physical therapists speech therapists and other highly trained professionals. This category is generally a fair assessment of a facility's rehabilitation services. Considering its grade in this category, we weren't surprised to discover that this nursing home is well below average in terms of the number of registered nurse and physical therapy hours offered to its residents based on the measures we looked at. Lastly, we considered the number of residents who were able to return home from this nursing home. This facility didn't fare well here either. In fact, we found that just 4 percent of this facility's residents were able to return home.
