Facility Inspections
B-
Historical
While this nursing home's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a grade of B- for that category, which is one of our better scores. We take several aspects of a facility's inspection report into consideration in determining our inspection ratings. One of those factors is deficiencies. You should keep in mind that the severity of these deficiencies is arguably more meaningful than the number of deficiencies, as some are quite minor. Fortunately, although this facility had some minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are labeled as categories G, H, I, J, K and L. This means that the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.
Long-term Care Quality
B-
Historical
This nursing home also didn't fare too badly in the category of long-term care. In fact, we awarded it a B- in that category, which wound up being another one of its better grades. Our long-term care ratings are based on a facility's amount of care. This includes a vast range of personal care services, ranging from assistance with activities of daily living to routine healthcare services. One of the criteria we considered after nurse's aid hours is vaccinations. Thankfully, this facility administered the pneumonia vaccination to 99.34211 percent of its patients. This place was able to keep its residents out of the hospital. In fact, it had less than one hospitalization per one thousand long-term resident days, which is an impressively low number.

Nurse Quality
D
Historical
We were surprised to find that this nursing home also excelled in the area of nursing. We gave it an impressive grade of D for that category, which is one of our highest grades. Our nursing grade includes quite a few components, but the most important consideration is the quantity of nurse hours spent with patients. This particular nursing home provided 3.7 hours of nursing care per resident per day, which is below average. On top of its weak nursing hour totals, this place received poor scores in several of the quality-based metrics we looked at in determining our nursing ratings. We found that a higher percentage of this nursing home's residents suffered falls leading to major injury. We would argue that many falls could likely be prevented with better nursing care. This statistic contributed to this place receiving a terrible nursing grade.
Short-term Care Quality
D
Historical
The next area we scored was short-term care, which proved to be a weak link for this nursing home. We gave this nursing home a grade of D in short-term care. In computing our short-term care scores, we look at the facility's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our objective is to devise a measure for sizing up the rehabilitation services of nursing homes. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Lastly, we looked at the percentage of patients who returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 0 percent of this facility's residents were able to return home.
