Facility Inspections

Grade: A

While we were not high on this facility overall, we awarded it an A for our inspections rating. This score is far more impressive than the nursing home's overall grade. These inspection ratings take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by obtaining copies of nursing homes' inspection reports. Although this facility had some deficiencies on its report, none of them were severe based on CMS' deficiency scale. A couple minor deficiencies aren't necessarily the end of the world.

Short-term Care Quality

Grade: B-minus

This nursing home's short-term care rating ended up being one of its best scores. The nursing home was awarded a grade of B- in this area. Our short-term care ratings are based on the facility's quantity of highly-skilled skilled healthcare professionals. This includes a vast scope of services, ranging from registered nurses to physical and respiratory therapists, as well as other forms of therapy. Unfortunately, we found that this facility provided fewer physical therapist and registered nurse hours per resident than most other nursing homes. The tricky part is determining what effect this had on the quality of care. The last metric we looked at in this category is the percentage of residents who ultimately were able to return home from the facility. We found that just 5.4 percent of this facility's residents returned home.

Nurse Quality

Grade: D

Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. The nursing rating includes a handful of subcategories, most of which are associated with quantities of nurse staffing. This place provided just 0.3 hours of nursing care per patient on a daily basis. Finally, we also assessed some nursing quality-based metrics in computing our nursing grades. This facility performed well in the area of minimizing its residents' major falls and pressure ulcers. Many consider these datapoints to be reliable indicators of the quality of nursing care.

Long-term Care Quality

Grade: F

Moving on to the next area, this facility really did not perform well here either. With a poor grade of F in long-term care, this is about as terrible as it gets. Facilities that do not score well in long-term care often don't provide as much nursing care and also may be struggling in some of the areas of routine personal care we assessed. Once we finished looking at the quantity of care provided by nurses, we turned to the nursing home's vaccination data. We were a bit concerned this facility vaccinated a relatively low 87.109375 percent of its residents against pneumonia. Surprisingly, this place was actually able to limit hospitalizations. With only 0.01 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than the majority of nursing homes. This was its best feature in this category.

Grade: A-plus

In Meadow View Health and Rehabilitation, 13.37% of Patients use Anti-Psychotic Medication

This tells you the percentage of patients who were prescribed antipsychotic drugs. Antipsychotic medications are given to patients for many conditions, such as dementia. This datapoint tells you the percentage of long-term care patients taking antianxiety medications.

Short-term Care: Rehospitalizations

Short-term Care: Facilitates Functional Improvement

Grade: B

In Meadow View Health and Rehabilitation, 73.53% Percentage of Resident

This is the percent of short-term patients who experienced functional improvements, such as with activities of daily living. High levels of independence with ADL's usually correlates with better rehabilitation services.
