Facility Inspections

Grade: B-minus

While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. These inspection grades take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by obtaining copies of nursing homes' inspection reports. Fortunately, although this facility had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G through L. This tells you the government inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies aren't the end of the world.

Nurse Quality

Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. Our nursing rating is primarily based on a facility's nurse staffing. Lastly, we also looked at some nursing quality-based metrics in computing our nursing ratings. This place fared well in the area of avoiding pressure ulcers and major falls.

Sadly, this facility received an F for its short-term care grade. In calculating our short-term care ratings, we assess the nursing home's levels of skilled nursing services, which includes registered nurses, physical therapists occupational therapists and other licensed professionals. This grade is considered to be a useful measure of a nursing home's ability to rehabilitate patients. Given its score in this category, we weren't surprised to find that this place is well below average in terms of the number of registered nurse and physical therapy hours offered to its patients based on the figures we assessed. Lastly, we considered the number of residents who were able to return to the community from this facility. We found that just 0 percent of this nursing home's residents returned home.

Turning to our next area, this facility didn't fare very well in this area either. With a rock bottom grade of F in long-term care, this is just about as terrible as it gets. With only 0 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than many nursing homes. This was its best score in this category.
