Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a grade of C for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades weigh a host of factors found on a facility's inspection report. One key criteria we consider is the quantity and severity of deficiencies. This nursing home had 5 deficiencies on its inspection report, but none were category G through L deficiencies. This means this facility avoided the more serious categories of deficiencies. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.
Short-term Care Quality
This facility's next most favorable category is short-term care, but it nevertheless earned just a D in that category. This is still a well below average score. Our short-term care grade is often used to measure a facility's rehabilitation services. To provide highly scored rehabilitation services, facilities generally must provide higher levels of skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other types of therapists. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The final item we considered in this area is the percentage of patients who were able to leave the nursing home and return to the community. We discovered that just 5.1 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics doomed this nursing home's short-term care score.

Long-term Care Quality
This nursing home ended up receiving a rock-bottom score in our long-term care area. It was given a lowly F in this area. Nursing homes that don't score well in this category often don't provide as much nursing care and also may be struggling in some of the areas of routine personal care we looked at. After we finished looking at the volume of nursing care, we turned to the facility's vaccination records. We were a bit worried that this nursing home vaccinated a relatively low 66.49916 percent of its residents for pneumonia. Surprisingly, this facility was actually decent at keeping its patients out of the hospital. Indeed, it had just 0 hospitalizations per one thousand long-term resident days. This was its best feature in this category.
Nurse Quality
The next category we assessed is nursing, where this facility received a very poor score in this category also. Unfortunately, we gave it an abysmal F for this category, which is definitely a major concern. Our nursing grade assesses the nursing home's level of nurse staffing. We look at both the levels of training of the nurses and the number of hours spent with patients. With a meager 0.3 hours of nursing care per resident per day, this place's nurse staffing levels were far below average. On top of receiving below average marks for total nursing hours, this place was a bit less impressive in some of the quality-based measures we looked at in determining our nursing ratings. We looked at the percent of residents suffering pressure ulcers and found this place was at approximately 1.5 times the national average in this metric. This is a bad sign when you consider that many bed sores are believed to be preventable with better nursing care. This statistic really pulled down this nursing home's nursing grade significantly.
