Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. Inspection grades are tied to many items located in the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these inspections. Severe deficiencies indicate a threat to the well being of residents. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Long-term Care Quality
We were also surprised to find that this facility received a decent long-term care grade in our assessment. We awarded it a grade of B- in that area. Our long-term care grade assesses a nursing home's personal care rather than the highly skilled nursing and medical services that are critical to a rehabilitation facility. One of the datapoints we considered in addition to nursing hours was vaccines. Candidly, we were a bit bothered by the fact that this facility provided the pneumonia vaccination to a relatively low 72.72727 percent of its patients. On a more positive note, this place was able to limit hospitalizations. In fact, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low figure.

We gave this facility a grade of just D for our nursing rating. Our nursing rating is based on numerous subcategories. The most heavily weighted variable is the amount of time nurses spend with patients. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Finally, we also looked at some nursing quality measures in calculating our nursing grades. This place fared well in terms of minimizing its residents' pressure ulcers and major falls. We consider these datapoints to be reliable indicators of the quality of nursing care.
Short-term Care Quality
This facility's least impressive category was short-term care, which is the final area we analyzed. For this category, we awarded this nursing home a grade of D. In determining our short-term care grades, we assess a facility's levels of skilled nursing services, which includes registered nurses, physical therapists occupational therapists and other types of therapists. This grade is considered to be a meaningful assessment of the facility's ability to rehabilitate patients. Given its score in this area, we were not stunned to learn that this place was well below average in terms of its number of registered nurse and physical therapy hours offered to its patients based on the measures we looked at. Finally, we assessed the percentage of residents that who were able to eventually return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 4.8 percent of this facility's residents were able to return home. This figure was well off the national average.
