Facility Inspections

Grade: A

On top of performing well in its overall grade, this facility performed at an even higher level in the area of inspections. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our highest grades in that category with a score of A. Inspection scores weigh several factors, including deficiencies, substantiated complaints and federal fines. You can find more information about each of these factors by obtaining copies of nursing homes' inspection reports. This nursing home was assessed 2 deficiencies on its inspection report, but none of the deficiencies were considered to be severe deficiencies. This indicates that the inspectors did not consider any of these deficiencies to be an imminent threat to resident safety or health. Keep in mind that deficiency-free inspections are uncommon in this industry.

Short-term Care Quality

Grade: B

This nursing home received an impressive short-term care grade as well. We gave it one of our better scores in that area with a grade of B. In the category of short-term care, we try to assess measures of a facility's rehabilitation. We look at a nursing home's skilled nursing services, including those performed by registered nurses and physical, occupational and other therapists. This nursing home employs both physical therapists and registered nurses. Not all facilities employ these skilled professionals. The final metric we assessed in this category is the number of patients that were able to return home from the facility. We found that it fared as well as just about any nursing home in Ohio in this area with 59.2 percent of its residents returning home. With most facilities, fewer than half of their short-term residents are able to return home.

Long-term Care Quality

Grade: B

This facility was given a favorable long-term care score as well. We gave it one of our better grades in that category with a score of B. Facilities that receive this type of grade in this category tend to provide consistent 24/7 care to make sure residents are kept in good health. After considering the quantity of care provided by aids and other staff, we next considered the nursing home's vaccination statistics. This nursing home vaccinated 99.53052 percent of its patients for pneumonia, which is significantly higher than the vast majority of nursing homes. Clearly, this facility is doing something right in this area as it also excelled at keeping its residents out of the hospital. Indeed, it had only 0 hospitalizations per 1,000 long-term resident days, which is an impressively low figure.

Nurse Quality

Grade: C

Moving on to our final area, this facility was given an acceptable nursing score of C. Our nursing score is mostly associated with a nursing home's nurse staffing. This facility provided just 3 hours of nursing care per patient daily. This is a low total compared with most other facilities. Finally, our nursing grades also consider some quality-focused measures. We like to look at the percentage of residents who experienced pressure ulcers and major falls. These data points are measures of the quality of nursing care, since superior care tends to reduce these issues.
