Facility Inspections
While this nursing home's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a C for that category, which is one of our better scores. We weigh several aspects of a nursing home's inspection report in computing these inspection scores. One key factor is deficiencies. It should be noted that the severity of the deficiencies is arguably more meaningful than the number of deficiencies, as some deficiencies can be relatively insignificant. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Unfortunately, we also need to draw your attention to the fact that this facility received some government fines in recent years.
Long-term Care Quality
Turning to the category of long-term care, this facility received a grade of just D in that category. Sadly, this proved to be one of its better features. If you are looking for services other than short-term rehabilitation, you should take a close look at each nursing home's long-term care ratings. On top of looking at the amount of care provided by aids and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This facility administered the vaccine to 98.7152 percent of its residents. To our surprise, this facility was able to limit hospitalizations. With just 0.03 hospitalizations per one thousand long-term resident days, this nursing home has less hospitalizations than the majority of nursing homes. Sadly, a few of its other scores in the category weren't as favorable as these.

Short-term Care Quality
This facility's second lowest category was short-term care, where it received a grade of just D in that category. This is a well below average grade. In the area of short-term care, we seek to qualify measures of a facility's rehabilitation. We assess the facility's skilled nursing services, including the ones performed by registered nurses and physical, occupational and other therapists. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last metric we looked at in this area is the percentage of patients who were able to leave the facility and return to the community. We discovered that just 4.8 percent of this facility's residents were able to return home, which is well below average. The combination of these poor metrics doomed this nursing home's short-term care score.
Nurse Quality
The final category we assessed was nursing, was this facility's worst category. We gave this facility a grade of D in this area. There are many factors included in this grade. Most of these data points reflect the quantity of nurse staffing. With a meager 0.4 hours of nursing care per patient on a daily basis, this nursing home's nurse staffing levels were significantly below average. On top of receiving below average marks for total nursing hours, this nursing home was less impressive in some of the quality-based metrics we looked at in determining our nursing scores. We looked at the percent of patients suffering pressure ulcers and found that this nursing home was at roughly 1.5 times the national average in this metric.
