This facility surprisingly performed well in long-term care. We gave it an A- for that area. For residents in need of a permanent residence rather than rehabilitation, long-term care is an important category. In addition to looking at the volume of care provided by aids and other staff, we analyzed the percentage of residents vaccinated for pneumonia. This facility gave the vaccine to 91.03139 percent of its residents, which is a few points less than we expected. The last datapoint we looked at is the facility's hospitalization rate. We found that this facility had 1.86 hospitalizations per one thousand long-term resident days, which is close to average in this area.

We also wanted to point out that this nursing home received an F for its nursing grade. We scrutinized the levels of nurses at the nursing home, in addition to the number of hours the nurses were with patients, in computing our score in this area. This facility averaged just 2.7 hours of nursing care per patient on a daily basis. This is a very low total compared to most nursing homes. To go along with its low totals in the area of nursing hours per patient, this facility had really bad marks in some of the quality-based measures we looked at in calculating our nursing ratings. We looked at the percentage of residents having falls which led to serious injury. This facility was at more than 1.5 times the national average in this statistic.

We wanted to note that this nursing home received a lowly F for its inspection score. An F in this category is alarming because it generally points to red flags on the facility's government inspection reports. We would advise paying special attention to any deficiencies on a nursing home's inspection reports when it receives this bad of an inspection rating. We would scrutinize the severity of the deficiencies. This facility had 4 deficiencies by CMS. The only positive thing we found is that none of the deficiencies were in the categories that suggest they posed a threat to resident safety or health. Finally, this nursing home was assessed large government fines recently.

The last category we assessed was short-term care, where this nursing home was given an abysmal score in this area as well. We gave it an abysmal F for this area, which is obviously a major disappointment. Our short-term care grade is typically used to gauge a facility's rehabilitation services. In order to offer quality rehabilitation services, nursing homes generally need to have higher levels of skilled nursing. These types of services include registered nurses, physical therapists, respiratory therapists and other licensed professionals. Considering its grade in this area, we weren't stunned to find that this facility is well below average in terms of the number of physical therapy and registered nurse hours offered to its patients based on the metrics we looked at. Frankly, this was about what we expected to find. Finally, we assessed the number of patients who were able to return to the community from this facility. This wasn't a strength for this nursing home. We found that just 40.4 percent of this facility's residents returned home. At most facilities, around half of their patients are able to return home so this is well below average.
