Facility Inspections
Sadly, we gave this place some really bad grades. This includes an F for its inspection grade. This is an especially important area. Scores in this range often point to some red flags on the facility's inspections. When a nursing home has this poor of an inspection rating, you should look for severe deficiencies on its inspection report. Severe deficiencies indicate a risk to the well being of residents. This facility was assessed 6 deficiencies on its inspection report. The only positive thing we can say is that none of these deficiencies were in the categories that suggest they caused a threat to resident safety or health. Finally, we also would like to draw your attention to the fact that this place received significant government fines in recent years in excess of $50,000. This usually signifies that CMS inspectors became frustrated after several years of bad government inspections.
Short-term Care Quality
Unfortunately, we gave this facility an F for its short-term care grade. Our short-term care scores are probably most important for those requiring rehabilitation from their nursing home. Rehabilitation usually requires additional highly-skilled nursing. This means not just nursing services, but also physical and respiratory therapy, as well as other forms of therapy. Given its weak score in this area, we weren't shocked to discover this nursing home offers substantially fewer registered nurse and physical therapist hours per resident than most other nursing homes. Finally, we looked at the percentage of patients who were able to return to the community from this nursing home. We found that just 3.8 percent of this nursing home's residents returned home.

Long-term Care Quality
Tragically, we gave this facility an F for its long-term care grade. Nursing homes that receive this type of grade in this category may not provide the kind of consistent around the clock care that some other facilities provide. After considering the amount of care provided by nurses and other staff, we next considered the nursing home's vaccination record. Frankly, we were a bit concerned this facility vaccinated 58.695652 percent of its residents for pneumonia, which happens to be quite a bit less than what we were hoping for. Surprisingly, this nursing home was actually decent at keeping its residents out of the hospital. It had only 0.01 hospitalizations per one thousand long-term resident days. This was its best feature in this category.
Nurse Quality
Turning to the next category, this nursing home did not perform very well here either. Due to a poor grade of F in nursing, this is about as as uninspiring as it gets. Our nursing grade looks at the facility's nurse staffing levels. We weigh both the levels of licensure of the nurses and the number of hours spent with patients. This place averaged only 0.2 hours of nursing care per resident on a daily basis. This is not a very impressive figure. In addition to rating below average in nursing hours per resident, this facility also had poor marks in the quality-based measures we looked at in calculating our nursing grades. We looked at the percent of patients experiencing pressure ulcers and major falls. This nursing home was at approximately 150 percent of the national average in both of these data points. This is a bad sign when you consider that many falls and bed sores are believed to be preventable with better nursing care. These statistics helped pull down this nursing home's nursing rating significantly.
