Facility Inspections
B-
Historical
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection scores account for several factors found on a nursing home's inspection reports. One key criteria we rely on is the number and severity of deficiencies. While this facility had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are ones labeled as categories G, H, I, J, K and L. This means the government inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Long-term Care Quality
D
Historical
Moving on to the area of long-term care, this nursing home received just a D in this area. As it turns out, this wound up being its second best grade. In a long-term care setting, the primary goal is to maintain patients' quality of life and keep them safe. On top of looking at the quantity of care provided by nurses aids and other staff, we analyzed the number of residents vaccinated against pneumonia. Thankfully, this facility vaccinated 94.63807 percent of its residents, which is a very respectable datapoint. Finally, we looked at its number of hospitalizations. Here we found that this facility fared radically worse with 4.47 hospitalizations per one thousand long-term resident days. Sadly, this figure is more than twice the national average.

Nurse Quality
F
Historical
We also wanted to draw your attention to this facility's poor nursing grade where it received an F. Our nursing rating weighs a number of datapoints. The most heavily weighted factor is the quantity of hours nurses spent with patients. This facility provided only 2.3 hours of nursing care per resident each day. This is well below average. Lastly, we also looked at some quality measures in determining our nursing scores. This nursing home was above average in two of the major datapoints we focus on, with good marks for avoiding major falls and pressure ulcers. These metrics are usually reliable indicators of the quality of nursing care provided.
Short-term Care Quality
F
Historical
The last category we graded was short-term care. We gave it a lowly F for this category, which is a bottom of the barrel score. In determining our short-term care ratings, we quantify the nursing home's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. The purpose is to formulate a tool for comparing the rehabilitation services of different nursing homes. With a grade of this caliber in this category, we were not shocked to learn this nursing home was well below average in multiple staffing figures we focus on. It offered substantially fewer registered nurse and physical therapist hours per resident than most other nursing homes. The last datapoint we looked at in this category is the number of residents that were able to return home from the nursing home. This nursing home struggled quite a bit in this metric as well, with just 22.3 percent of its residents returning home.
