Facility Inspections
While we were not high on this nursing home overall, we awarded it a B+ for our inspections category. This grade is far better than the place's overall score. Our inspection ratings weigh a host of factors included in a facility's inspection reports. One key criteria we consider is the number and severity of deficiencies. You should avoid facilities with too many deficiencies flagged. Although this place had some deficiencies on its inspection report, none were major deficiencies based on CMS' deficiency scale. A couple minor deficiencies are not necessarily the end of the world.
Short-term Care Quality
Moving on to the category of short-term care, this nursing home was given just a D in that category. Unfortunately, this wound up being its second best grade. In computing these short-term care ratings, we look at the nursing home's skilled nursing services, such as those performed by registered nurses, respiratory therapists, physical therapists and other types of therapists. Our purpose is to create a measure for sizing up the rehabilitation services of different nursing homes. As you might expect, we learned that this place offers far less registered nurse and physical therapist hours per patient than the typical nursing home. The final measure we considered in this category is the number of patients that ultimately returned home from the facility. This nursing home struggled quite a bit in this metric as well, with just 4.9 percent of its patients returning home.

Long-term Care Quality
Sadly, this nursing home only received a grade of D for its long-term care score, which is not a very good score. When facilities receive this kind of grade in long-term care it is typically a bad sign for patient care and it may indicate that the facility is not as well-staffed. After considering the quantity of care provided by nurses aids and other staff, we next considered the nursing home's vaccination data. This facility vaccinated 92.349724 percent of its patients against pneumonia, which happens to be a bit lower than we anticipated but still a respectable figure. Surprisingly, this facility was actually able to limit hospitalizations. With just 0.02 hospitalizations per one thousand long-term resident days, this facility had less hospitalizations than most nursing homes. This is its best score in this category.
Nurse Quality
This facility's least impressive category was nursing, which is the last area we scored. For this area, we gave this nursing home a D. Our nursing score is primarily tied to a facility's level of nurse staffing. This nursing home averaged just 0.3 hours of nursing care per patient daily. This is an alarmingly low total. To pair with its below average totals in the area of nursing hours per patient, this nursing home had poor scores in some of the quality-based measures we looked at in determining our nursing ratings. For starters, we looked at the percentage of patients having falls which led to major injury. This facility was at more than 150 percent of the national average in this statistic.
