Facility Inspections
Although this place's overall grade was as bad as it gets, it actually performed pretty well in the area of inspections. In fact, we awarded it a B+ for that category, which is one of our better scores. These inspection grades weigh several factors, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by reviewing copies of nursing homes' inspection reports. Although this facility had some deficiencies on its government inspection report, none of them were severe based on CMS' scale. A few minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
We awarded this facility just a D for our long-term care grade and that actually ended up being its second best score. When nursing homes receive this kind of grade in this category it is often a bad sign for patient care and it may mean that the nursing home is not as well-staffed with nurses and aids. After looking at the quantity of care provided by nurses and other staff, we then analyzed the nursing home's vaccination data. Candidly, we were a bit alarmed this facility vaccinated 72.125435 percent of its patients for pneumonia, which is quite a few points below what we expected. Surprisingly, this nursing home was actually able to limit hospitalizations. With only 0.01 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than many nursing homes. This was its best score in this category.

Nurse Quality
Turning to the area of nursing care, this facility received a grade of just D in this category. Our nursing grade is mostly based on a facility's nurse staffing. This facility provided only 0.3 hours of nursing care per patient per day. Lastly, we also looked at some nursing quality-based metrics in computing our nursing ratings. This place fared well in the area of avoiding pressure ulcers and major falls. Many consider these areas to be good indicators of the quality of nursing care.
Short-term Care Quality
Switching gears to the final area, this nursing home really didn't perform very well in this category either. Due to a poor grade of F in short-term care, this is about as as uninspiring as it gets. In calculating these short-term care grades, we assess the facility's skilled nursing services, such as those performed by registered nurses, speech therapists, physical therapists and other types of therapists. Our purpose is to devise a measure for sizing up the rehabilitation services of various nursing homes. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Lastly, we looked at the number of patients that ultimately returned home from this facility. This facility didn't fare well here either. In fact, we found that just 4.1 percent of this facility's residents were able to return home. This figure was well off the national average.
