Facility Inspections

Grade: A-plus

One of the many reasons this turned out to be a quality nursing home is that it received an A+ inspection grade. This is simply as good as it gets in this critical category. Perhaps the most significant factor we consider in determining our inspection scores is deficiencies. Deficiencies are found on a nursing home's inspection reports. Nursing homes with better scores in this category most likely dodged the most severe deficiencies involving things like patient abuse. Although this nursing home had a few deficiencies on its report, none of them were serious based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Short-term Care Quality

Grade: A-minus

Among its many impressive scores, this facility was given a top-shelf short-term care score. In fact, we gave it a grade of A- in that area. Our short-term care grade is typically employed to gauge a facility's performance with rehabilitation. To provide highly rated rehabilitation services, nursing homes generally need to offer better levels of skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other types of therapists. Fortunately, this nursing home offers both physical therapists and registered nurses. Not all facilities offer both. The final item we assessed in this category is the number of residents who ultimately returned home from the facility. This place fared as well as just about any nursing home in Minnesota in this area with 63 percent of its residents returning home. For most nursing homes, fewer than half of their short-stay residents are able to return home.

Nurse Quality

Grade: A-minus

This nursing home also received a very impressive nursing grade. In that area, we awarded this facility a grade of A-. Our nursing score assesses many datapoints, most of which are associated with quantities of nurse staffing. This nursing home provided 4.5 hours of nursing care per resident per day, which was among the better figures we found. Lastly, we also looked at nursing quality measures in determining our nursing scores. These include minimizing residents' pressure ulcers and major falls. We look at these statistics as reliable indicators of the quality of nursing care being provided, since better care tends to reduce these problems.

Long-term Care Quality

Grade: B-plus

Turning to the fourth area, this facility was given a very strong long-term care rating. We awarded it a grade of B+ in that area, topping off one of our more favorable report cards. Nursing homes that excel in this category typically provide residents with better supervision and stay on top of routine medical care. In addition to offering very impressive levels of nurse staffing, this nursing home administered the pneumonia vaccine to 98.68421 percent of its residents. This percentage is also far better than the majority of nursing homes. This combination proved to be effective as this nursing home was able to keep its residents out of the hospital. Indeed, it had only 0.45 hospitalizations per one thousand long-term resident days, which is an impressively low number.
