Long-term Care Quality
This nursing home didn't perform quite as poorly in the area of long-term care. We awarded it a grade of B- for that category. Our long-term care grade focuses on a nursing home's personal care instead of the skilled therapy and healthcare services that are critical to a rehabilitation facility. After assessing the amount of care provided by aids and other staff, we turned to the facility's vaccination statistics. We were a bit worried that this facility vaccinated 77.67296 percent of its residents against pneumonia, which happens to be several points below the national average. On a more positive note, this facility was able to limit hospitalizations. Indeed, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low figure.
Nurse Quality
We gave this facility a grade of just D for our nursing rating and that actually turned out to be its second highest score. There are a host of subcategories included in this area. Most of the datapoints reflect staffing levels. This place averaged a meager 0.3 hours of nursing care per resident each day. This is an alarmingly low total. Lastly, we also looked at some nursing quality-based metrics in calculating our nursing ratings. This facility performed well in terms of minimizing its residents' pressure ulcers and major falls. We consider these areas to be reliable measures of the quality of nursing care.
Does this page answer your questions about Landmark of Louisville Rehabilitation and Nursing?
Facility Inspections
Unfortunately, this nursing home received very weak inspections in recent years, causing us to give them an F for inspections. An F in this category is typically a signal that we found some red flags on the facility's inspections. For places with poor inspection scores, you should really scrutinize any severe deficiencies found on their inspection reports. Tragically, this nursing home was hit with several category J through L deficiencies. This indicates multiple widespread deficiencies putting resident health or safety in imminent jeopardy. These deficiencies are toxic to a nursing home's inspection grade. In addition to having severe deficiencies, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Short-term Care Quality
The last area we assessed is short-term care, where this nursing home was given an abysmal score in this category also. Sadly, it received a lowly F for this category, which is obviously somewhat alarming. Our short-term care ratings are important for individuals requiring rehabilitation. Rehabilitation typically mandates more skilled nursing. This means a vast scope of services, spanning from registered nurses to physical and respiratory therapists, as well as other types of therapy. As you might expect, we learned that this nursing home provides substantially less physical therapist and registered nurse hours per resident than the average facility. Lastly, we assessed the percentage of patients who were able to return to the community from this nursing home. This definitely was not a strength for this nursing home. We found that just 4 percent of this nursing home's residents returned home. At most facilities, around half of their patients are able to return home so this is well below average.
