Facility Inspections
The primary reason this facility received a decent overall score is its excellent inspections in recent years. We awarded them one of our higher grades in that category, with an A+. This is a much better score than the facility's overall grade, which was decent but not in the elite range. We take several aspects of a facility's inspection report into consideration in determining our inspection scores. One critical factor is deficiencies. You should keep in mind that the severity of deficiencies is arguably more important than the quantity of deficiencies, as some of these end up being relatively insignificant. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Based on this, we had positive view of this nursing home's inspections.
Short-term Care Quality
This facility was given its next best category grade in the category of short-term care. We gave this nursing home a grade of C in this area, which is a decent grade. In our short-term care grade, we strive to forge a meaningful barometer for rehabilitation. In this process, we assess a nursing home's levels of highly-skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. Unfortunately, we found that this facility provided fewer registered nurse and physical therapist hours per resident than most other nursing homes. The more important thing is determining what bearing this had on quality of care. The last statistic we considered in this category is the percentage of residents who eventually returned home from the facility. We found that just 4.7 percent of this nursing home's patients returned home. This figure was below average.

Long-term Care Quality
As it turns out, this nursing home only earned a D for its long-term care score, which is not one of our better scores. In a long-term care setting, the facility's primary goal is to maintain residents' quality of life and keep them safe. One of the factors we considered on top of nurse's aid hours was vaccinations. Thankfully, this facility administered the pneumonia vaccine to 100 percent of its patients. To our surprise, this nursing home was able to limit hospitalizations. With only 0.01 hospitalizations per 1,000 long-term resident days, this nursing home has fewer hospitalizations than most nursing homes. Sadly, a few of its other scores in this category weren't as favorable as these.
Nurse Quality
The last category we graded is nursing, which turned out to be this facility's worst area. We gave this nursing home a D in that area. Our nursing rating consists of many factors, many of which are tied to nursing hours. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Unfortunately, this facility also had terrible scores in some of the quality-based measures to pair with its weak nursing hour totals. We looked at the percentage of residents sustaining pressure ulcers. This nursing home was at more than twice the national average in this datapoint. This is concerning when you consider that so many pressure ulcers are preventable with better nursing care. This ended up sealing the nursing home's fate of receiving a poor nursing rating.
