To pair with its relatively modest overall score, we awarded this nursing home a grade of A for our inspections rating. This score is far more impressive than its overall grade. Inspection ratings weigh several factors included in a nursing home's inspection report. One of the most important criteria we rely on is the quantity and severity of deficiencies. Facilities with higher grades in this area tend to have few severe deficiencies. Although this facility had a few deficiencies on its government inspection report, none of them were severe based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is always a good sign.
Long-term Care Quality
Another one of this nursing home's more favorable category grades was in the category of long-term care. In that area, we gave this nursing home a B+. If you are looking for anything other than short-term rehabilitation, you should take a close look at long-term care grades. Once we assessed the amount of care provided by nurses and other staff, we then analyzed the nursing home's vaccination record. This facility vaccinated 92.82051 percent of its residents for pneumonia, which is slightly less than we were hoping for but still a respectable percentage. Lastly, this facility was able to limit hospitalizations. With only 1.03 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than most nursing homes.

Moving on to another strength for this facility, it also earned an elite nursing grade. In fact, we gave this nursing home an D. There are numerous criteria included in this area. Most of these datapoints are tied to levels of nurse staffing. This facility averaged only 2.7 hours of nursing care per resident on a daily basis. This is an alarmingly low total. On top of its weak nursing hours, this facility also received poor scores in some of the quality-based measures we looked at in calculating our nursing ratings. We found that a higher percentage of this place's patients suffered falls leading to major injury.
Short-term Care Quality
The last category we looked at is short-term care, which proved to be this facility's weakest category. We gave it a lowly F for this category, which is obviously concerning. Our short-term care ratings are crucial for individuals looking for rehabilitation. Rehabilitation generally mandates additional highly-skilled nursing. Skilled nursing means a wide scope of nursing services, ranging from registered nurses to physical and occupational therapists, as well as other variations of therapy. Considering its grade in this category, we were not stunned to learn that this nursing home was well below average in terms of its quantity of physical therapy and registered nurse hours provided to its residents based on the measures we assessed. Honestly, this was about what we expected here. The final item we looked at in this category is the percentage of patients who returned home from the facility. This facility struggled quite a bit in this area as well, with just 32 percent of its residents returning home. This was quite a bit below the national average.
