Facility Inspections
While this place's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a B- for that category, which is one of our better scores. We believe that one of the most critical factors in evaluating an inspection report is deficiencies. In this regard, we especially focus on the severity of the deficiencies. This facility received 3 deficiencies on its inspection report, but none were considered severe. This indicates that the inspectors didn't deem any of these deficiencies to cause an immediate risk to resident health or safety. A few minor deficiencies aren't the end of the world.
Long-term Care Quality
We awarded this nursing home a grade of just D for our long-term care rating and that actually wound up as its next highest category. If you are seeking anything other than rehabilitation, you should take a close look at long-term care scores. After considering the volume of care provided by nurses and other staff, we then looked at the facility's vaccination record. We were optimistic to learn that this facility vaccinated 96.192894 percent of its residents against pneumonia. To our surprise, this nursing home was able to limit hospitalizations. With just 0.04 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than the average nursing home. Sadly, some of its other scores in the category were not as favorable as these.

Short-term Care Quality
We gave this nursing home just a D for our short-term care grade. This is not a score to write home about. Our short-term care ratings are crucial for individuals seeking rehabilitation. Rehabilitation typically requires additional highly-skilled nursing. This means a vast range of services, ranging from registered nurses to physical and respiratory therapists, as well as other variations of therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last metric we looked at in this area is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 4.4 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics sunk this facility's short-term care grade.
Nurse Quality
Turning to the fourth area, we gave this facility a grade of D for the area of nursing. This was the facility's least impressive category score. Our nursing category analyzes a handful of factors, many of which are based on levels of nurse staffing. This nursing home provided just 0.3 hours of nursing care per patient daily. This is well below average. To go along with its low totals in the area of nursing hours per patient, this place also did not perform as well in the quality-based measures we looked at in calculating our nursing ratings. We looked at the percent of patients experiencing major falls and pressure ulcers. This nursing home had more falls and pressure ulcers than the average facility.
