Facility Inspections
While this place's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a B- for that category, which is one of our better scores. Inspection ratings take several factors into consideration, including deficiencies and federal fines. You can find more information about each of these items by obtaining copies of nursing homes' inspection reports. Fortunately, although this place had a few minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This tells you that the inspectors did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, CMS flagged this nursing home for possible abuse or neglect. This is not what you want to see. We advise you to direct your search elsewhere.
Long-term Care Quality
We were also pleasantly surprised to discover that this nursing home received a reasonably solid long-term care score in our assessment. We gave it a grade of B- in this category. If you are looking for anything other than rehabilitation, you should take a close look at long-term care ratings. In addition to looking at the volume of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This facility provided the vaccine to 100 percent of its patients, which is more than the vast majority of nursing homes. Finally, this facility was also able to limit hospitalizations. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low figure.

Nurse Quality
This facility's next best category was nursing, but even there it received a grade of just D. This is still a well below average grade. The nursing rating features a variety of factors. The most heavily weighted one is the quantity of hours nurses spent with patients. This facility provided just 0.4 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also assessed some nursing quality-based metrics in computing our nursing grades. This facility fared well when it comes to minimizing its residents' major falls and pressure ulcers. Many consider these statistics to be measures of the quality of nursing care.
Short-term Care Quality
Switching gears to the final category, this facility really did not perform well in this area either. Due to an abysmal grade of F in short-term care, this is about as terrible as it gets. In forming these short-term care grades, we look at the nursing home's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. Our goal is to create a measure for sizing up the rehabilitation services of different facilities. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we assessed the number of residents that ultimately returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 3.2 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
