Integrity Hc of Carbondale is a large facility located in Carbondale, Illinois. This nursing home received an F, which is really a bottom of the barrel score in our grading scheme. If you are not happy with this facility's poor overall grade, you may find your options to be limited in Carbondale. The city has just one other nursing home. We were surprised to find that this nursing home actually received one respectable category grade for inspections, which you can find more information about below.

Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Our inspection ratings are tied to pieces of information found on the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. Severe deficiencies indicate a risk to resident safety. While this nursing home had some deficiencies on its government inspection report, none were major deficiencies based on CMS' scale. A few minor deficiencies aren't necessarily the end of the world.
Nurse Quality
One of the other reasons we graded this nursing home so highly is that it received a very impressive nursing grade. In that area, we awarded this nursing home an F. In determining our nursing ratings, we weigh both staffing levels and the levels of licensure of the nurses. This nursing home provides 2.5 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Unfortunately, this nursing home also had terrible scores in some of the quality-based measures to pair with its low nursing hour totals. We looked at the percentage of residents suffering pressure ulcers and we were disappointed. This place was at roughly 150 percent of the national average in this metric. This is a bad sign when you consider that so many bed sores could be avoided with better nursing care. This metric helped pull down this nursing home's nursing score significantly.
Long-term Care Quality
The next area we analyzed is long-term care. This facility's was given a low-end long-term care grade where it received an F. For patients seeking a permanent place to live as opposed to skilled nursing, long-term care grades are an important measure. Once we looked at the volume of care provided by nurses and other staff, we turned to the nursing home's vaccination records. This nursing home vaccinated 99.58678 percent of its patients against pneumonia, which is better than most nursing homes. Unfortunately, this place's hospitalization rate was far less impressive. We found that this nursing home had 2.48 hospitalizations per one thousand long-term resident days, which is an alarming rate.
Short-term Care Quality
The last area we scored was short-term care. It received an F for this category, which is an abysmal grade. Our short-term care scores are probably more critical for folks needing rehabilitation from their nursing home. Rehabilitation usually mandates more skilled nursing services. This means not only nursing services, but also physical and speech therapy, as well as other forms of therapy. Not surprisingly, we found that this place offers significantly less registered nurse and physical therapist hours per patient than the typical nursing home. Finally, we looked at the number of patients who were able to return to the community from this facility. This wasn't a strength for this nursing home. We found that just 28.2 percent of this facility's residents returned home. At most nursing homes, about half of their patients are able to return home so this is well below average.
