Facility Inspections
This nursing home earned a solid overall grade as described above, but we really wanted to point out its excellent inspection reports in recent years. We gave them an impressive grade of A in this category. Perhaps the most significant factor we look at in calculating our inspection ratings is deficiencies. Deficiencies can be found on a nursing home's recent inspection reports. Facilities with higher scores in this category typically avoided the most severe deficiencies involving health risks or even death. While this place had a few minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are those found in categories G, H, I, J, K and L. This means the inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple of minor deficiencies should not lead you to cross a nursing home off your list.
Long-term Care Quality
We also gave this nursing home a middle of the road grade in the category of long-term care, where we gave it a grade of C. For long-term care residents, the facility's primary objective is to maintain patients' quality of life and keep them safe. On top of considering the quantity of care provided by aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This nursing home administered the vaccine to 87.473 percent of its patients. This is several points lower than what we were hoping for. On a more positive note, this nursing home was able to limit hospitalizations. Indeed, it had less than one hospitalization per one thousand long-term resident days, which is an impressively low figure.

Short-term Care Quality
We gave this nursing home a grade of just D for our short-term care rating. This is not a very good score. Our short-term care scores are based in part on a facility's quantity of skilled nursing services. This means a broad range of nursing services, spanning from registered nurses to physical and occupational therapists, not to mention other forms of therapy. With its grade in this area, we weren't stunned to find that this place was well below average in terms of its quantity of physical therapy and registered nurse hours provided to its residents based on the metrics we assessed. Frankly, this was about what we expected here. Finally, we considered the percentage of patients who were able to return to the community from this facility. This was not a strength for this facility. We found that just 5.2 percent of this nursing home's residents returned home. At most nursing homes, around half of their residents are able to return home so this is well below average.
Nurse Quality
The next area we assessed was nursing, which ended up being a weak link for this facility. We gave this facility a grade of D in our nursing category. There are several factors within this category. Many of these data points reflect staffing levels. This place averaged just 0.3 hours of nursing care per resident on a daily basis. This is a very low total compared to most nursing homes. In addition to its subpar nursing hour totals, this facility received poor marks in many of the quality-based metrics we considered in computing our nursing grades. We found that more of this place's patients suffered falls leading to major injury.
