Facility Inspections
Although we didn't give this facility a good overall score, we did give it a strong grade in the category of inspections, where it received a B+. Arguably the most critical factor we consider in determining our inspection scores is deficiencies. These deficiencies can be found on a facility's recent inspection reports. It is typically best to avoid facilities that had too many severe deficiencies. While this place had a few minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This means that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies aren't the end of the world.
Long-term Care Quality
This facility was given its second most favorable category grade in the category of long-term care. We awarded this facility a grade of B- in this area, which is a reasonably acceptable score. In a long-term care setting, the nursing home's primary objective is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. After considering the amount of care provided by aids and other staff, we next considered the nursing home's vaccination data. This nursing home vaccinated 100 percent of its patients for pneumonia, which is better than the majority of nursing homes. Lastly, this nursing home was also able to limit hospitalizations. In fact, it had only 0.04 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.

Nurse Quality
This facility's next best category was nursing, but even there it received a grade of just D. This is still a well below average grade. Our nursing score analyzes many factors, however, the main consideration is the quantity of nurse hours per patient per week. This place provided only 0.4 hours of nursing care per resident on a daily basis. This is well below average. Lastly, we also assessed some nursing quality-based metrics in determining our nursing ratings. This place fared well in the area of minimizing its residents' major falls and pressure ulcers.
Short-term Care Quality
Moving on to our last area, this facility didn't perform very well here either. Due to a rock bottom grade of F in short-term care, this is about as as uninspiring as it gets. In forming these short-term care ratings, we quantify the facility's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. The purpose is to formulate a tool for comparing the rehabilitation services of various facilities. With a grade this poor for this category, we weren't surprised to discover this nursing home was well below average in many staffing areas we focus on. In fact, it provided significantly fewer registered nurse and physical therapist hours per resident than most facilities. The final metric we considered in this category is the percentage of residents who were able to leave the facility and return to the community. We discovered that just 3.8 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar metrics doomed this nursing home's short-term care score.
