Facility Inspections
While this nursing home's overall grade was as bad as it gets, it actually really excelled in the area of inspections. In fact, we gave it a grade of A- for that category, which is one of our highest scores. We take several aspects of a facility's inspection report into consideration in determining our inspection grades. One key factor is health deficiencies. You should keep in mind that the severity of the deficiencies is usually more important than the quantity of deficiencies, as some of these are relatively minor. Although this facility had a few deficiencies on its inspection report, none were major deficiencies based on CMS' scale. A couple minor deficiencies aren't the end of the world.
Long-term Care Quality
Moving on to the area of long-term care, this nursing home was given a grade of just D in that area. Unfortunately, this proved to be one of its better grades. When nursing homes receive this type of grade in long-term care it is often a bad sign for resident care and it may indicate that the place is not as well-staffed with nurses and aids. On top of considering the volume of care provided by aids and other staff, we also looked at the number of residents vaccinated for pneumonia. Candidly, we were a bit alarmed this nursing home gave the vaccine to a relatively low 75.47893 percent of its residents. Surprisingly, this nursing home was actually decent at keeping its patients out of the hospital. Indeed, it had just 0.02 hospitalizations per 1,000 long-term resident days. This was its best feature in this category.

Short-term Care Quality
Unfortunately, we gave this facility an F for its short-term care score. In calculating our short-term care scores, we analyze the facility's levels of highly skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other types of therapists. This area is typically a useful assessment of the nursing home's rehabilitation services. Considering its grade in this area, we weren't surprised to find that this nursing home is well below average in terms of its number of registered nurse and physical therapy hours provided to its residents based on the measures we looked at. Lastly, we looked at the number of residents who returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 3.4 percent of this facility's residents were able to return home.
Nurse Quality
The final area we looked at was nursing. We gave it a lowly F for this area, which is a poor score. When determining our nursing grades, we assess both nursing hours and the skill levels of the nurses. This nursing home offered just 0.3 hours of nursing care per resident on a daily basis. This number is far below average. This place also had abysmal scores in some of the quality-based metrics to go along with its low nursing hours. For starters, we looked at the percent of residents sustaining pressure ulcers. This nursing home was at more than double the national average in this statistic. This is alarming when you consider that many pressure ulcers are preventable with better nursing care. This ended up sealing the nursing home's fate of receiving a very poor nursing rating.
