This usually means that inspectors became frustrated after several years of bad health inspections.

This nursing home wound up receiving a rock-bottom score in our long-term care category. It was given a lowly F in this area. Facilities that receive this kind of grade in long-term care likely don't provide the type of consistent around the clock care that some other facilities offer. Candidly, we were a bit bothered by the fact that this nursing home administered the pneumonia vaccine to only 48.387096 percent of its patients. This was its best score in this category.

Based on its grade in this category, we were not surprised to discover that this facility was well below average in terms of its quantity of registered nurse and physical therapy hours provided to its residents based on the measures we assessed. Frankly, this was what we expected here. Lastly, we assessed the percentage of residents who were able to return home from this facility. This facility didn't fare well here either. In fact, we found that just 2.8 percent of this facility's patients were able to return home.
