Holston Manor is a very-large nursing home located in Kingsport, Tennessee. We awarded it an overall grade of B-, which is a middle of the road score. At the end of the day, this facility should be a solid choice for quite a few people. The best part of this place's profile was its stellar inspection reports. Facility Inspections
This nursing home received a solid overall grade as described above, but we really wanted to emphasize its excellent health inspection reports in recent years. We gave them an A+ in this category. Inspection scores weigh several factors, including deficiencies and federal fines. You can learn more about each of these items by obtaining copies of nursing homes' inspection reports. Fortunately, although this facility had some minor dings on its report, it had zero severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This means that the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple relatively minor deficiencies aren't something to panic about.
Long-term Care Quality
Another one of this facility's higher category scores was in the category of long-term care. In that category, we gave this facility a B+. Nursing homes that receive this type of score in long-term care typically provide consistent around the clock care to ensure patients are kept in good health. Once we assessed the amount of care provided by nurses aids and other staff, we then looked at the nursing home's vaccination data. This nursing home vaccinated 100 percent of its patients against pneumonia, which is significantly higher than the average nursing home. Finally, this nursing home was able to limit hospitalizations. With just 1.26 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than the average nursing home.
Short-term Care Quality
The third area we analyzed was short-term care, where this nursing home was given a grade of C in this category. In the category of short-term care, we seek to evaluate measures of a nursing home's rehabilitation. We look at the facility's skilled nursing services, such as the ones performed by registered nurses and various therapists. This nursing home employs both physical therapists and registered nurses. Not all facilities employ these skilled professionals. The final item we considered in this area is the number of residents who were able to leave the facility and return to the community. We discovered that just 41.7 percent of this facility's residents were able to return home, which is actually below average.

The next area we rated was nursing, which is this facility's worst area. We gave this facility a D in this area. Sadly, this grade pulled down the facility's overall profile a bit. Nursing grades are based in large part on nurse staffing levels. This nursing home provides 3.2 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. In addition to its subpar nursing hour totals, this facility also received suboptimal scores in several of the quality-based measures we looked at in computing our nursing grades. We found that more of this place's patients suffered falls leading to serious injury.
