Facility Inspections

Grade: A

To pair with its relatively modest overall grade, we awarded this nursing home an A for its inspection grade. This grade is far better than its overall score. Perhaps the most important factor we look at in calculating our inspection scores is deficiencies. These deficiencies can be found on a nursing home's recent inspection reports. Nursing homes with better grades in this area most likely avoided the more severe deficiencies involving patient abuse or death. Although this facility had some deficiencies on its government inspection report, none of them were major deficiencies based on CMS' scale. A couple of minor deficiencies shouldn't stop you from considering a nursing home.

Long-term Care Quality

Grade: C

This nursing home's next best area was its long-term care score, where it was given a C. Our long-term care scores are more meaningful for people in need of more conventional assistance related to activities of daily living. In addition to looking at the amount of care provided by nurses and other staff, we looked at the number of residents vaccinated for pneumonia. This nursing home provided the vaccine to 97.82016 percent of its patients. Finally, this place was also able to limit hospitalizations. In fact, it had only 0.03 hospitalizations per one thousand long-term resident days, which is a remarkably low number.

Short-term Care Quality

Grade: C

The third area we graded was short-term care. In that category, we awarded this facility a grade of C. In the category of short-term care, we strive to assess measures of a facility's rehabilitation. We analyze a nursing home's skilled nursing services, such as those performed by registered nurses and physical and occupational therapists. We found that this place was below average in the two key staffing areas we focus on. The facility supplied fewer physical therapist and registered nurse hours per resident than most other facilities. The last datapoint we considered in this category is the percentage of patients who were able to leave the facility and return to the community. We discovered that just 4.8 percent of this facility's patients were able to return home, which is actually below average.

Nurse Quality

Grade: D

This facility's least impressive category is nursing, which is the final area we assessed. For this category, we awarded this facility a grade of D. There are a host of subcategories within this grade. Most of these criteria relate to nurse staffing. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. In addition to receiving below average scores for total nursing hours, this place was less impressive in some of the quality-based measures we looked at in calculating our nursing ratings. We looked at the percentage of residents suffering pressure ulcers and found this facility was at approximately 1.5 times the national average in this statistic.

Hillcrest Village Quality Metrics

Minimizes Pressure Ulcers

Grade: B

In Hillcrest Village, 7.87% of Patients had Pressure Ulcers

This tells you the percent of residents that suffered from a pressure ulcer. Pressure ulcers, which are also called bed sores, are often the result of patients not being moved frequently enough. Better nursing protocols reduces the percentage of residents in a nursing home who sustain pressure ulcers.

Minimizes Serious Falls

Grade: B-plus

In Hillcrest Village, 2.18% of Patients had Serious Falls

This tells you the percent of patients who have had a major fall. Falls resulting in severe injuries are often the result of lower levels of patient supervision. Additional nurse staffing can limit the number of major falls sustained by residents in a nursing home.

Minimizes Urinary Tract Infections

Grade: D

In Hillcrest Village, 4.79% of Patients had UTIs

This metric is an indication of the percent of long-term stay patients who had a UTI. While a higher rate of these infections could reflect poorly on a facility's nursing care, it is problematic to compare between nursing homes due to inconsistent reporting standards.

Appropriately Uses Anti-Psychotic Medication

Grade: A

In Hillcrest Village, 7.53% of Patients use Anti-Psychotic Medication

This tells you the percent of residents who were prescribed antipsychotic drugs. While antipsychotic drugs may be helpful for many patients, it is important to ensure these drugs are being used appropriately. In some situations, increased usage of these drugs may mean a nursing home is using these drugs to control patient behavior.

Appropriately Uses Anti-Anxiety Medication

Grade: C

In Hillcrest Village, 25.07% of Patients use Anti-Anxiety Medication

This metric is an indication of the percent of long-term patients taking antianxiety medications.

Managing Depression Among Residents

Grade: D

In Hillcrest Village, 21.58% of Patients

This metric is a measure of the percentage of long-term care residents demonstrating depressive symptoms. Some argue this is a measure of patient quality of life.

Appropriate Vaccine Usage

Grade: B-plus

In Hillcrest Village, 98.91% of Patients

This is the percent of residents who received the flu and pneumonia vaccines. Higher vaccination rates should be the norm at this point.

Residents Maintain Autonomy

In Hillcrest Village, 14.37% of Patients

This is the percent of patients who needed increased assistance with activities of daily living over time.

Ability to Keep Residents Mobile

Grade: F

In Hillcrest Village, 2.14% of Residents

This is the percent of patients that maintained mobility over time. Optimizing mobility is often a great sign for residents' health.

Hospitalizations

Grade: D

In Hillcrest Village, 0.03 Hospitalizations per 1,000 resident days

This is a measure of the number of hospitalizations per thousand days of patient care. Avoiding the hospital is important to maintaining the physical health of nursing home patients.

Short-term Care: Rehospitalizations

Grade: A-plus

In Hillcrest Village, 2.14% of Residents Rehospitalized

This is a measure of the number of times residents are rehospitalized per 1,000 days of short-term patient care.

Short-term Care: ER Visits

Grade: B

In Hillcrest Village, 11.03% of Patients

This tells you the number of times residents are sent to the emergency room per 1,000 days of short-term care. There is usually a correlation between keeping patients out of the emergency room and the quality of care.

Short-term Care: Facilitates Functional Improvement

Grade: C

In Hillcrest Village, 72.03% of Resident

This is a measure of the percent of short-term patients who saw functional improvements.
