Facility Inspections
Although we graded this facility incredibly poorly overall, it actually earned a very impressive government inspection report this year. As a result, it earned one of our better grades in that category with a B+. Inspection grades weigh a host of factors found on a facility's inspection reports. One of the most important criteria we rely on is the quantity and severity of deficiencies. You generally want to avoid nursing homes with a bunch of severe deficiencies flagged. This particular nursing home received 5 deficiencies on its inspection report. Overall, this is still a solid inspection report, but we'd always recommend looking into any deficiencies you find troubling.
Long-term Care Quality
We also would like to note that this facility received an F for its long-term care grade. Nursing homes that don't fare well in this category often do not provide as much nursing care and also may be lagging in some of the areas of routine personal care we looked at. After considering the amount of care provided by nurses aids and other staff, we then looked at the facility's vaccination statistics. We were concerned by the fact that this nursing home vaccinated a relatively low 22.605364 percent of its patients for pneumonia. Surprisingly, this nursing home was actually able to limit hospitalizations. With only 0.01 hospitalizations per one thousand long-term resident days, this nursing home had less hospitalizations than many nursing homes. This was its best feature in this category.

Short-term Care Quality
In addition, we gave this nursing home a rock-bottom F in the category of short-term care. In calculating our short-term care scores, we size up a facility's levels of highly skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other highly skilled individuals. This grade is generally a useful assessment of a nursing home's rehabilitation services. Not surprisingly, we learned that this facility offers substantially less physical therapist and registered nurse hours per resident than the typical facility. Lastly, we assessed the percentage of patients who were able to return to the community from this facility. This definitely wasn't a strength for this facility. We found that just 5 percent of this facility's residents returned home. At most nursing homes, about half of their patients are able to return home so this is well below average.
Nurse Quality
The next category we looked at was nursing, in which this nursing home was given an abysmal score here as well. It received an F in this area, which is definitely very concerning. There are a host of subcategories within this category. Many of these factors are tied to levels of nurse staffing. This nursing home provided just 0.3 hours of nursing care per resident on a daily basis. This is well below average. Unfortunately, this nursing home also had abysmal scores in some of the quality-based measures to pair with its low nursing hour totals. We looked at the percentage of residents experiencing pressure ulcers and we were disappointed. This place was at approximately 1.5 times the national average in this measure. This is a bad sign in light of the fact that so many pressure ulcers are preventable with better nursing care. This statistic helped pull down this nursing home's nursing score significantly.
